Approve and authorize the Director of the Department of Social Services or designee to sign Amendment No. 4 to Agreement No. A-16274 with Central Coast Center for Independent Living to provide housing navigation/stabilization and housing-related financial assistance for older adults and persons with disabilities who are receiving services from Adult Protective Services (APS) or are in the APS intake process and who are at risk of homelessness or experiencing homelessness, adding $277,653 for a new contract total of $1,278,572 and extending the term by thirteen (13) months for a new agreement term of May 10, 2023 through June 30, 2027; and b. Authorize the Director of the Department of Social Services or designee to sign up to three amendments to this Agreement where the total amendments do not exceed 10% ($127,857) of the amended contract amount, do not significantly change the scope of work, and do not exceed the maximum aggregate amount of $1,406,429.Tue, 14 Apr 2026 22:33:51 GMT
