RECOMMENDATION:
It is recommended that the Board of Supervisors:
Approve and authorize the Director of Health or Assistant Director of Health to execute Amendment No. 2 to Standard Agreement A-13961 with Mark William Stanford, Ph.D., Consultant for the provision of training and consultation services to extend the term for two (2) additional years for a revised term of July 1, 2018 to June 30, 2022 and to increase funds in the amount of $25,500, for a revised total Agreement amount not to exceed $51,099.

SUMMARY/DISCUSSION:
Mark William Stanford, Ph.D. ("Dr. Stanford"), Consultant, has over twenty-five (25) years in behavioral science education and is retired from the Santa Clara Valley Health and Hospital System where he was the Director of the Addiction Medicine and Behavioral Therapy services. Dr. Stanford was also a psychotherapist for many years at the Psychiatric Medical Group in San Jose, California specializing in the treatment of depression and anxiety disorders. Dr. Stanford is a past affiliate clinical faculty member at Stanford University School of Medicine in the Department of Psychiatry and Addiction Medicine and is currently an Associate Professor of Neuroscience and Psychology at Cabrillo College in Aptos, California. Dr. Stanford is an Associate Member of the American Society of Addiction Medicine (ASAM) and a certified trainer in the ASAM Criteria.
