a. Approve and authorize the Director of Health or Assistant Director of Health to execute a Mental Health Services Agreement for the term retroactive to March 4, 2019 through June 30, 2023 with Koinonia Foster Homes, Inc. DBA Koinonia Homes for Teens for the provision of mental health services in the amount of $883 for Fiscal Year (FY) 2018-19; $26,401 for FY 2019-20; $324,247 for FY 2020-21; $324,247 for FY 2021-22; and $324,247 for FY 2022-23, for a total Agreement amount not to exceed $1,000,025; and
b. Approve and authorize the Director of Health or Assistant Director of Health to approve up to three (3) future amendments that do not exceed ten percent (10%) ($100,002.50) of the original Agreement amount and do not significantly change the scope of services.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
