Approve and authorize the Director of Health or Assistant Director of Health to execute a Renewal Agreement and Amendment No. 3 to Agreement A-13138 with Central Coast Center for Independent Living (CCCIL) to extend the Rapid Rehousing Access provisions of the Whole Person Care (WPC) Program 2 an additional Calendar year to December 31, 2021, adding $366,960 of available WPC funding, while also extending CCCIL Program 1 counseling and outreach services as a non-renewal component of the Amendment an additional fiscal year through FY 2021-22, adding $133,103 of MHSA funding, for a new maximum County obligation of $2,112,978, for a revised term of July 1, 2016 through June 30, 2022.

It is recommended that the Board of Supervisors:
