a. Approve and authorize the Director of the Department of Social Services to sign Amendment #1 to the agreement with Coalition of Homeless Service Providers for homeless services coordination and Whole Person Care integration to extend the term through December 31, 2021, add $331,145.29 for a contract total of $944,353.29; and
b. Authorize the Director of the Department of Social Services to sign up to three (3) amendments to the agreement that do not exceed 10% ($94,435) and do not significantly change the scope of services.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Approve and authorize the Director of the Department of Social Services to sign Amendment #1 to the agreement with Coalition of Homeless Service Providers for homeless services coordination and Whole Person Care integration to extend the term through December 31, 2021, add $331,145.29 for a contract total of $944,353.29; and
b. Authorize the Director of the Department of Social Services to sign up to three (3) amendments to the agreement that do not exceed 10% ($94,435) and do not significantly change the scope of services.
SUMMARY/DISCUSSION:
This amendment allows for the continuation of homeless services coordination in conjunction with the Department of Health Care Services' Whole Person Care (WPC) program. WPC began as a pilot program through the State Department of Health Care Services with the goal of coordinating health, behavioral health, and social services to improve the health and wellbeing of Medi-Cal recipients. The State plan is to transition from Whole Person Care to the CalAim Program. The Monterey County Health Department is the lead agency for the County and DSS has partnered with them in the operation of this program.
