RECOMMENDATION:

It is recommended that the Board of Supervisors:

a. Approve and authorize the Director of the Department of Social Services to sign an agreement with the Central Coast Center for Independent Living for $120,527 to provide enhanced information and assistance services to all ages, disabilities, and income for the period of August 1, 2020 through June 30, 2021; and

b. Authorize the Director of the Department of Social Services to sign up to three (3) amendments to this Agreement where the total amendments do not exceed 10% ($12,053) of the original contract amount and does not significantly change the scope of work.

 

SUMMARY/DISCUSSION:

Central Coast Center for Independent Living is a local non-profit that provides a variety of programs to assistant people with disabilities. They have been working with the Area Agency on Aging (AAA) and the Monterey County Department of Social Services, Aging and Adult Services Branch (AAS) to form a public/non-profit partnership. The purpose of this agreement is to promote increased collaboration amongst service providers, locate shared resources, and create an efficient delivery of services to assist the community in responding to increasing service demand and shrinking public resources.

 

The California Department of Aging has recognized this partnership and has provided funding to this emerging collaboration following an Aging and Disability Resource Connection (ADRC) model. The approach is designed to better serve older adults and individuals with disabilities by allowing better access to information, services and supports.

 

This Agreement provides the following:  1) Enhanced Information & Assistance, 2) Person Centered Options Counseling, 3) Short Term Service Coordination, 4) Person Centered Transition Support Services, 5) Lead agency to assist individuals with disabilities, 6) Consumer-directed resource for long term support and services (LTSS), 7) Promote and implement a “no wrong door” approach to provide services in a seamless and easy way, and 8) Bring existing resources together to empower consumers to make information decisions about their LTSS.

 

OTHER AGENCY INVOLVEMENT:

The Auditor-Controller and Purchasing have reviewed and approved this agreement. County Counsel has approved the agreement as to form.

 

 

FINANCING:

Aging and Disability Resource Connection (ADRC) funding is provided by the California Department of Aging. Sufficient appropriations and estimated revenues for SOC010 are included in the FY 2020-21 Adopted Budget. 

 

BOARD OF SUPERVISORS STRATEGIC INITIATIVES:

This Agreement correlates to the Health & Human Services Strategic Initiative adopted by the Board of Supervisors by working towards the goals of improving health and quality of life outcomes by providing outreach and supportive services to seniors.
