Title

Accept a grant award in the amount of $1,262,057 from the California Department of Health Care Services (“DHCS”) California Advancing and Innovating Med-Cal (CalAIM) Providing Access and Transforming Health (PATH) Capacity and Infrastructure, Transition, Expansion and Development (CITED) grant program, for the grant term of July 1, 2024, through June 30, 2025; and

b. Ratify execution by the Director of Health Services of the Acknowledgement of Grant Terms and Conditions, an express condition of the County’s receipt of grant funds from the DHCS CalAIM PATH CITED grant program, to support activities to build and expand infrastructure and capacity and implement Enhanced Care Management and Community Supports at health clinics, for a total grant amount of $1,262,057, and for a total grant term of July 1, 2024, through June 30, 2025; and

c. Approve and authorize the Director of Health Services or designee to sign up to three (3) future amendments to this Agreement where the amendments do not exceed 10% ($126,205) of the grant agreement amount or significantly change the scope of work, for a maximum amount of $1,388,263.

RECOMMENDATION:

It is recommended that the County of Monterey Board of Supervisors:

a. Accept a grant award in the amount of $1,262,057 from the California Department of Health Care Services (“DHCS”) California Advancing and Innovating Med-Cal (CalAIM) Providing Access and Transforming Health (PATH) Capacity and Infrastructure, Transition, Expansion and Development (CITED) grant program, for the grant term of July 1, 2024, through June 30, 2025; and

b. Ratify execution by the Director of Health Services of the Acknowledgement of Grant Terms and Conditions, an express condition of the County’s receipt of grant funds from the DHCS CalAIM PATH CITED grant program, to support activities to build and expand infrastructure and capacity and implement Enhanced Care Management and Community Supports at health clinics, for a total grant amount of $1,262,057, and for a total grant term of July 1, 2024, through June 30, 2025; and

SUMMARY/DISCUSSION:

The County of Monterey Health Department (MCHD) has been awarded a competitive one-year grant from DHCS. This grant, offered through the CalAIM PATH CITED Program, is a significant opportunity for MCHD.

 

The CalAIM PATH CITED grant program aims to support California's efforts to expand and strengthen its capacity for implementing the CalAIM initiative targeting a wide range of stakeholders, including community-based organizations, public hospitals, county agencies, and others. Importantly, it places particular emphasis on historically under-resourced organizations, which aligns with the goal of advancing health equity and addressing social determinants of health.

 

One of the primary objectives of the CalAIM PATH CITED grant program is to provide funding for building the necessary capacity and infrastructure to collaborate effectively with managed care plans. This collaboration is vital for the successful implementation of the CalAIM Enhanced Care Management (ECM) and Community Supports (CS) programs, which adopt a holistic, community-based approach to serving individuals with complex health and social needs.

 

The County of Monterey Health Department has successfully secured $1,262,057 in funding through the CalAIM PATH CITED Round 3 grant from the DHCS. This grant, effective from July 1, 2024, to June 30, 2025, will support several critical initiatives. It will establish a Community Hub for CalAIM ECM/CS billing and case management, focusing on agencies that provide services in the areas of birth equity and prenatal health. Additionally, the grant will enable the implementation of a school-based ECM program targeting foster youth and families experiencing homelessness. Furthermore, this funding will include the launch of an Asthma Remediation Program and a recuperative care program.

 

The Director of Health Services executed the Acknowledgement of Grant Terms and Conditions from the California Advancing and Innovating Medi-Cal (CalAIM) Providing Access and Transforming Health (PATH) Capacity and Infrastructure, Transition, Expansion, and Development (CITED) grant program to secure the grant funding by the DHCS deadline. There was no Board of Supervisors meeting scheduled between the receipt of the Acknowledgement of Grant Terms and Conditions and the signing deadline.

 

This work supports the County of Monterey’s Health Department’s 2018-2024 Strategic Plan Goals: 1.  Empower the community to improve health through programs, policies, and activities; 2.  Enhance community health and safety by emphasizing prevention; and 3. Ensure access to culturally and linguistically appropriate, customer-friendly, quality health services. It also supports one or more of the ten essential public health services, specifically:  3. Inform, educate, and empower people about health issues; 4. Mobilize community partnerships and action to identify and solve health problems; 7. Link people to needed personal health services and assure the provision of health care when otherwise unavailable. 

 

OTHER AGENCY INVOLVEMENT:

The Offices of County Counsel and Auditor-Controller have reviewed and approved the Award and Agreements as to form and fiscal provisions, respectively.

 

FINANCING:

There is no impact to the General Fund resulting from the approval of this action.  Due to the timing of the grant award, the related program revenue and expenditures are not included in the MCHD’s FY 2024-25 Adopted Budget (001-4000-8600-HEA014). The Department will evaluate the need to increase appropriations and will bring any necessary actions to the Board for consideration and approval.
