Approved and ratified the action taken by the Director of Health and the
Contracts/Purchasing Officer to sign an Agreement with the Blue Shield of California
Foundation for Grant #4572769, in the amount of $200,000, beginning January 1, 2011
through December 31, 2011, for the purpose of implementing the 2010 County Coverage
Expansion Planning, Round Two.

SUBJECT: Approve and ratify the action taken by the Director of Health and the
Contracts/Purchasing Officer to sign an Agreement with the Blue Shield of
California Foundation for Grant #4572769, in the amount of $200,000, beginning
January 1, 2011 through December 31, 2011, for the purpose of implementing the
2010 County Coverage Expansion Planning, Round Two.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
Approve and ratify the action taken by the Director of Health and the Contracts/Purchasing
Officer to sign an Agreement with the Blue Shield of California Foundation for Grant #4572769,
in the amount of $200,000, beginning January 1, 2011 through December 31, 2011, for the
purpose of implementing the 2010 County Coverage Expansion Planning, Round Two.
SUMMARY/DISCUSSION:
California currently reimburses ten 10) counties for services provided to medically indigent adults
through the Section 1115 Medi-Cal Waiver's County Coverage Expansion Planning, Round One.
The Patient Protection and Affordable Care Act, signed into law on March 23, 2010, calls for
federal health care reform, including an expansion of Medicaid Medi-Cal) coverage for
individuals with income up to 133% of the Federal Poverty Level by 2014. As a result, California
is seeking to expand the program and accepted new Round Two Section 1115 Waiver
Demonstration Proposals. The Coverage Initiative Expansion Program presents new counties with
the opportunity to pave the way to increase health care coverage to county residents who will be
affected by the provisions of federal health care reform legislation. The mandated expansion of
Medi-Cal eligibility is projected to result in 23,452 Monterey County residents becoming eligible
for Medi-Cal by 2014.
On August 18, 2010, the Interim Director of Health and Natividad Medical Center Chief Executive
Officer notified the Board of Supervisors and the County Administrative Officer of the intent to
submit an application for grant funding to the Blue Shield of California Foundation BSCF") to
support the planning and development of a county coverage initiative expansion proposal under the
provisions of the Section 1115 Medi-Cal Waiver's Health Care Coverage Initiative program.
On December 8, 2010, the BSCF notified the Health Department that it had awarded a grant to
Monterey County. In order to obtain funding for 2010, the signed Agreement was required to be
returned by December 17, 2010. A memo to the Board advising of the action taken was submitted
on December 16, 2010 Attachment 1). Due to the time constraint to comply with this requirement,
the Director of Health signed the Agreement, and the Agreement was circulated for appropriate
approvals. County Counsel, Risk Management, the Auditor-Controller, and the
Contracts/Purchasing Officer reviewed and approved the Agreement Attachment 2).
The process will convene key stakeholders and accelerate collaboration to position Monterey
County to expand coverage to currently uninsured residents in ways that are sustainable. The
Coverage Initiative planning process will enable Monterey County to analyze relevant data and

to healthcare coverage for uninsured Monterey County residents.
OTHER AGENCY INVOLVEMENT:
County Counsel, Risk Management, the Auditor-Controller and Contracts/Purchasing have
reviewed and approved the Agreement as to legal form, insurance requirements, and fiscal
provisions. Agreement and memo to the Board are on file with the Clerk of the Board.
FINANCING:
Blue Shield of California Foundation will fund the grant in the amount of $200,000 for the period
January 1, 2011 through December 31, 2011; the Health Department will provide an in-kind match
of $115,000, Natividad Medical Center will provide an in-kind match of $179,000 and the
Department of Social Services will provide an in-kind match of $12,000. The grant does not
require a cash match. The recommended board action does not" include an increase to the
revenue and expenditure budgets at this time even though these are new funds previously not
included in the FY 2011 Adopted Budget. The budget adjustment will be deferred to later in the
fiscal year if needed as directed by the County Administrative Office Budget and Analysis
Division. There will be no increase in General Fund Contribution due to the acceptance of this
grant.
