RECOMMENDATION:
It is recommended that the County of Monterey Board of Supervisors:
a. Approve and authorize the Director of Health Services or designee to execute an Amendment No.3 for the purchase of a four-year service agreement extension for the existing GeneXpert 4x4 Public Health Laboratory equipment, for a total Agreement amount not to exceed $225,409; and

b. Approve a non-standard Agreement as recommended by the Director of Health Services.

SUMMARY/DISCUSSION:
The Health Department’s Public Health Laboratory (PHL) is responsible for providing testing services to identify microorganisms that cause disease including bacteria, fungi, viruses, and parasites. The PHL provides testing for communicable disease surveillance purposes, surge events and other public health emergencies. The PHL also serves as a reference laboratory for local hospitals and health care agencies.

The County of Monterey Health Department is seeking approval to amend its current agreement with Cepheid to extend preventative maintenance (PM) services for its GeneXpert 4x4 instrument for a four (4) year annual preventative maintenance plan. PM service helps prevent equipment breakdown, lowers chances of premature instrument replacement, and ensures reliable and timely test results. The PHL uses its GeneXpert for rapid detection of infectious diseases such as Mycobacterium tuberculosis, SARS-CoV-2, influenza viruses, and the respiratory syncytial virus (RSV), which is critical for controlling the spread of disease in local communities.

This work supports the County of Monterey Health Department 2025-2028 Strategic Plan Goals:  2. Enhance public health and safety through prevention, and 4. Engage Health Department workforce and improve operational functions to meet current and developing population health needs. It also supports three of the ten essential public health services, specifically: 1. Assess and monitor population health status, factors that influence health, and community needs and assets; 2. Investigate, diagnose, and address health problems and hazards affecting the population, and 9. Improve and innovate public health functions through ongoing evaluation, research, and continuous quality improvement.

OTHER AGENCY INVOLVEMENT:
County Counsel and Auditor-Controller have reviewed this Agreement as to legal form and fiscal provisions, respectively.

FINANCING:
The amendment is funded 100% with Epidemiology and Laboratory Capacity (ELC) grant funds in FY 2025-26, and by 34% laboratory fee revenue and 66% Public Health Realignment for FY 2026-27, FY 2027-28, and FY 2028-29. Appropriations for this amendment are included in the Health Department’s (HEA003-8124) FY 2024-25 Adopted Budget and will be included in future Requested Budgets.
