RECOMMENDATION:

It is recommended that the Board of Supervisors:

a.                                          Authorize the Chief Probation Officer to accept a Providing Access and Transforming Health (PATH) Justice Involved Capacity Building Program Grant award in the amount of $500,000 from Public Consulting Group (the third party grant Administrative Entity for the California Department of Health Care Services (DHCS), to support collaborative planning with Department of Social Services (DSS) to implement the processes, protocols, and IT system modifications necessary to support the implementation of pre-release enrollment and suspension processes that will help ensure Medi-Cal coverage upon reentry into the community for justice-involved youth to ensure continuity of treatment for the grant term retroactive to October 27, 2023 through June 20, 2026;

SUMMARY/DISCUSSION:

The Department of Health Care Services (DHCS) received federal approval under its California

Advancing and Innovating Medi-Cal (CalAIM) 1115 Demonstration for Providing Access and

Transforming Health (PATH) capacity building funds to support the Justice-Involved Reentry Initiative.

 

Under the CalAIM 1115 Demonstration, DHCS received approval to disseminate up to $410 million in capacity building funds to support the planning and implementation of pre-release and reentry services in the 90 days prior to an individual’s release into the community. California is the first state in the nation to receive federal approval to provide pre-release services. The Monterey County Probation Department is among many county agencies Statewide that meet the eligibility for this grant. The County’s grant allocation specific to the Probation Department is $500,000.

 

The PATH Justice-Involved (JI) Reentry Initiative capacity building program will provide funding to support the planning and implementation of the provision of targeted pre-release Medi-Cal services to individuals in state prisons, county jails, and youth correctional facilities who meet the eligibility criteria.  This initiative aims to advance health equity and improve physical and behavioral health outcomes for the justice-involved population to avert unnecessary use of inpatient hospitals, psychiatric hospitals, nursing homes, emergency departments, and other forms of costly and inefficient care.

 

Monterey County Probation will ensure sustainability of pre-release enrollment and suspension processes using PATH JI funding through the potential programming of our case management system (CMS) to automate and streamline the current manual process, and through the development and modification of protocols and procedures in support of automation and streamlining in the CMS.  The existing vendor would provide programming support of the CMS applications to develop and modify the system.

 

OTHER AGENCY INVOLVEMENT:

Staff has consulted with the Office of County Counsel. Risk Management reviewed the grant

agreement due to non-standard insurance.

 

FINANCING:

There will be no cost to the County General Fund associated with the recommendation.  Additionally, there are no required increases in appropriations in the current fiscal year as grant activities are anticipated to begin in FY2024-25.  The department plans to incorporate grant related expenditures and offsetting reimbursement revenues into future budget cycles through FY 2025-2027.
