RECOMMENDATION:

 

It is recommended the Board of Supervisors:

 

a.  Adopt a resolution finding that the expenditure of funds from the Natividad Medical Center (NMC) Fiscal Year 2017/2018 Budget to support a Funding Agreement with Sun Street Centers (“SSC”), assisting SSC in meeting the direct costs of renovation of its residential detoxification facility (“RDF”) in Salinas, California, has a direct and primary public purpose, benefiting Monterey County, in general, and NMC, in particular;

b.  Authorize the Deputy Purchasing Agent for NMC or his designee to execute a Funding Agreement with SSC, to provide funding to SSC in an amount not to exceed $40,000 to assist in meeting the direct costs of the RDF renovation, for a term beginning September 1, 2017 through August 31, 2018.

 

SUMMARY/DISCUSSION:

 

For fifty years, SSC has operated a Men’s Residential Program, a state-licensed men’s residential social model program for recovery from substance use disorders.  Operating out of SSC’s Salinas, California, facility, CCS is the sole provider for social model withdrawal management/detoxification services as part of residential and outpatient alcohol and drug recovery services in Salinas and one of only two providers of this service in all of Monterey County. The other provider is located in Seaside, California. Social model services do not utilize medical personnel instead using substance abuse counselors and relying on the medical clearance of clients in hospitals like NMC before transport to SSC. SSC plans to open a Women’s Residential Program in King City next year.  

 

As part of SSC’s Men’s Residential Program, individuals undergo an initial 72 hour period of detoxification from alcohol or drugs in the RDF, a specifically designated area of SSC’s Salinas facility.  The RDF is separate from other treatment areas of the SSC and consists of rooms with a total of eight beds and a separate breakroom/living room. Each year, SSC serves approximately 260 residents at the RDF. Most of SSC’s residents are referred to SSC by Monterey County Health Department, Behavioral Health Services, Monterey County Probation Department, and Natividad. This last year, NMC had 144 patient referrals from the hospital to SSC for social model detoxification services following medical clearance at NMC primarily through the Emergency Department.

 

As a result of Medicaid Expansion in California as part of the Affordable Care Act, SSC now has the opportunity to implement a hybrid social model/medical model of recovery for its Men’s Residential Program, blending SSC’s traditional social model recovery components and Medi-Cal approved withdrawal management/detoxification services determined to be medically necessary by either a medical director or other licensed clinician. These Medi-Cal approved services include the performance on a resident’s intake of a physical examination and laboratory testing, clinical observation of a resident’s health status during withdrawal/detoxification, the provision to a resident of mental health services, and the prescription and administration of medication for the resident. With the implementation of a hybrid social model/medical model of recovery at SSC, patients will not require lengthy hospital emergency department visits prior to receiving care at SSC.  

To comply with licensing requirements of the State Department of Health Care Services for the operation of a residential recovery program with medical model withdrawal management and detoxification services, SSC has determined it must renovate its Salinas RDF (“RDF renovation”) in its detox area converting existing space into a private medical treatment and counseling room with quick access and easy supervision by medical personnel, enlarging the sleeping quarters, and expanding staff office space for medical personnel. The cost for this renovation is $45,000. $5,000 has been raised through fundraising.

 

OTHER AGENCY INVOLVEMENT:

 

County Counsel has reviewed and approved this Funding Agreement as to legal form and risk provisions, and the Auditor-Controller has reviewed and approved as to payment provisions. The Funding Agreement has also been reviewed and approved by NMC’s Board of Trustees on August 4, 2017.

 

FINANCING:

 

The cost for this Funding Agreement is $40,000, all of which will be absorbed in NMC’s Fiscal Year 2017-18 Adopted Budget. There is no impact to the General Fund.

 

BOARD OF SUPERVISORS STRATEGIC INITIATIVES:

 

For fifty years, SSC has operated a Men’s Residential Program, a state-licensed men’s residential social model program for recovery from substance use disorders.  Operating out of SSC’s Salinas, California, facility, CCS is the sole provider for withdrawal management/detoxification services as part of residential and outpatient alcohol and drug recovery services in Salinas and one of only two providers of this service in all of Monterey County. Access to this vital service improves health and quality of life for the residents served by SSC.
