RECOMMENDATIONS:
It is recommended that the Board of Supervisors adopt a Resolution:
 
a.      Accepting the Certificate of Completion for the Medical Surgical Unit Workroom and Office Refresh for Natividad Medical Center, located at 1441 Constitution Blvd., Salinas, CA 93906, Project No. 9600-26, Bid Package No. NMC-1000; and
 
b.      Authorizing the Assistant Administrator of Operations & Support Services to execute and record the Notice of Completion.
 
SUMMARY/DISCUSSION:
On March 27, 2012 the Board of Supervisors awarded a construction contract to Mill Construction, Inc, to complete the Medical Surgical Unit Workroom and Office Refresh Project for Natividad Medical Center (NMC), Salinas, CA. The project's scope of work included demolition of an existing toilet room, millwork, finishes, ceiling work, and mechanical, electrical, and plumbing work to 311 sq/ft. of a workroom. The toilet room was removed to allow for additional work space for the physicians. Second phase of work included removing and replacing carpet and wall finishes and demolition of a pass through window in the kitchen area in the Administrative Department.
 
The project started the week of May 29, 2012 and was completed by August 29, 2012. It was determined to have been satisfactorily performed in accordance with the plans and specification.
 
Mill Construction Inc.'s contract was in the amount of $135,749.00. There was an allowance of $20,000 included within Mill Construction's general contract. Six change orders were issued for a total of $12,438.00 of the allowance used, Mill Construction Inc.'s revised contract amount decreased by $7,562.00 for a new contract aggregate amount of $128,187.00. All work was accomplished within the approved contract amount and on schedule.
 
The contract is complete in all respects in accordance with the specifications. The work is ready to be accepted by the Board of Supervisors.
 
OTHER AGENCY INVOLVEMENT:
County Counsel has reviewed and approved this Resolution. The Resolution has also been reviewed and approved by Natividad Medical Center's Board of Trustees.
 
 
FINANCING:
There is no impact to the General Fund. The Medical Surgical Unit Workroom and Office Refresh project is fully funded by Capital Budget ID #B-2011-58. There were sufficient funds to cover project costs.  Final pay applications to Mill Construction Co. Inc. in the amount of $121,777.65 have been processed and a balance of $6,409.35 is due from retention.
