RECOMMENDATION:

It is recommended that the Board of Supervisors:

Approve and authorize the Director of Health or Assistant Director of Health to execute a no-extension Amendment No. 1 to Mental Health Services Agreement A-12686 with Crestwood Behavioral Health, Inc. for the provision of mental health services to Monterey County adult residents with severe psychiatric disabilities in the increased amount of $1,250,398 for Fiscal Year (FY) 2015-16, and $1,362,854 for FY 2016-17 (a 26.7% increase over the original Agreement) for a new total Agreement amount of $3,545,999 for the same full term of FY 2014-15 through FY 2016-17.

 

SUMMARY/DISCUSSION:

Crestwood Behavioral Health, Inc. has provided mental health services for approximately 12 years, and for 30 years prior under the previous corporate umbrella of Crestwood Hospitals.  Crestwood has 18 California facilities ranging in size from 12 to nearly 200 beds, seven of which are covered under the terms of this Agreement.  Crestwood's programs provide recovery oriented psychiatric services to adults, older adults, and transition-aged youth with severe and persistent mental health illnesses. Crestwood provides intensive treatment programs with a high level of security and education to assist clients to live independently.  Crestwood treatment services include an individualized program to meet the specific needs of each client, and a structured regimen with services to assist clients in their development of new skills.

 

This Amendment No. 1 to existing Agreement A-12686 increases the service rates by 3.5% per year for FY 2015-16 and FY 2016-17, the last two years of the three -year Agreement,  aligning Crestwood with industry rates as offered to Institutions for Mental Disease via AB 1054, and with other Mental Health Rehabilitation Centers and Skilled Nursing Facilities. 

 

This Amended Agreement continues past mental health services realigns the allocated service units from the original Agreement to more closely match actual patient usage, and expands support at key facilities in San Jose, Sacramento, and Fremont, providing the County of Monterey additional options in placing patients, while giving a total 26.7% increase over the original Agreement.   The Amendment also adds financing for an extended bed hold rate, providing flexibility to Monterey County patients and Program Managers.  Crestwood continues to provide services at a lower cost for those clients who may otherwise be referred to a State Hospital at a higher rate.

 

This Agreement contains the County’s standard 30-day “no cause” provision (Section IV, B) and an additional defunding provision (Section IV, D), which provides the County the ability to amend or terminate the Agreement in the event of a reduction and/or termination of funding.

This work supports the following Monterey County Health Department 2011-2015 strategic plan initiative: 3) Ensure access to culturally and linguistically appropriate, customer-friendly, quality health services.  It also supports one of the ten essential public health services, specifically: 7) Link people to needed personal health services and assure the provisions of health care when otherwise unavailable.

 

OTHER AGENCY INVOLVEMENT:

County Counsel and Auditor-Controller have reviewed and approved this Agreement as to legal form and fiscal provisions, respectively. A copy of this Amendment No. 1 to Agreement A-12686 is on file with Clerk of the Board.

 

FINANCING:

This Agreement is funded by Realignment revenues (100%).  The funds for this Agreement ($1,250,398) are included in the Health Department’s Behavioral Health (HEA012, Unit 8410) Fiscal Year 2015-16 Adopted Budget.  The funds for this Agreement for FY 2016-17 ($1,362,854) will be included in the Health Department’s Behavioral Health (HEA012, Unit 8410) Fiscal Year 2016-17 Requested Budget.
