RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Approve and authorize the Contracts/Purchasing Officer to sign Amendment #1 to Agreement A-10833 with California Forensic Medical Group, Inc. (CFMG) for increased staffing for the provision of Inmate Healthcare Services in the Adult Detention Facility of Monterey County;

SUMMARY/DISCUSSION:
The Sheriff's Office recommends amending Agreement A-10833 with California Forensic Medical Group (CFMG) to allow for increased medical staffing in the County Jail to respond to increases and changes to medical and mental health requirements of inmates due to impacts of the State Public Safety Realignment. CFMG is the current medical services provider for inmates at the County Jail. The services include medical, mental health and dental services. Changes in the inmate population due to Public Safety Realignment have caused the cost of providing medical and mental health care to the inmates of the Monterey County jail to go up dramatically. Because inmates are staying in the jail longer, the County has had to spend more time and money on inmates with chronic conditions such as diabetes and heart disease. The amendment to CFMG's contract recognizes the need for additional medical personnel staffing due to the effects of Realignment. The amendment to this agreement will provide the additional medical services of 1.0 FTE Physician's Assistant, 2.8 FTE Registered Nurse, 2.8 FTE Licensed Vocational Nurse, and 2.4 FTE Certified Nursing Assistant/MA. Additionally, the amendment will provide for additional mental health support with 0.62 FTE Psychiatrist (on-call) and 0.5 FTE Psychiatric RN/MFT.
To facilitate the increased provision of medical and mental health services of the additional CFMG staff, the Sheriff's Office recommends the addition of 11 Corrections Deputies to provide the necessary escort services for inmate to and from medical personnel; improved staff and inmate safety; and, transport services of inmate to the hospital and outside medical appointments. To accommodate the increased and changing health needs of inmates, the recommended increase in deputies will provide 10 medical escort deputies (3 deputies on both day-shift teams and 2 on both night shift teams) as well as an additional transportation deputy to transport and supervise inmates needing medical treatment outside of the County Jail.
OTHER AGENCY INVOLVEMENT:
The County Administrative Office, County Counsel, Probation and the Health Department have reviewed and concur with the recommendation.
FINANCING:
Approval of the Amendment to the Agreement A-10833 with CFMG to provide additional medical and mental health service in the County Jail will result in an increase cost of approximately $1,350,110. It is recommended to increase appropriations in Sheriff's Office Budget 2300, SHE004-8237 by $1,350,110. It is recommended to amend Sheriff's Office Budget 2300, SHE003-8238 and increase Appropriations by $1,534,205 to add 10 Deputy Sheriffs -Corrections in Jail Operations to facilitate the necessary medical escorts of inmates, to assist the CFMG medical staff, and to provide increased safety of inmate and jail staff. It is further recommended to amend Sheriff's Office Budget 2300, SHE003-8234 and increase appropriations by $153,421 to add 1 Deputy Sheriff -Corrections in the Transportation unit to ensure adequate staffing to provide transport and supervision of inmates requiring medical attention outside of the County Jail. Funding for this action will come from Health and Mental Health Realignment (Funds 023 and 025), unbudgeted Fund Balance from FY2013-14, and Public Safety Realignment Fund 022, subject to the approval of the Community Corrections Partnership (CCP). Implementation of the actions will be contingent upon availability and approval of all funding sources.
