Title
a. Accept and approve a Mobile Clinic Agreement template to be used in partnering with school districts throughout Monterey County for the delivery of primary medical care, oral health services, behavioral health services, health education, and other services by the County of Monterey Health Department Clinic Services Bureau to the Monterey County community; and

b. Approve and accept a second Mobile Clinic Agreement template to be used in partnering with school districts throughout Monterey County for the delivery of oral health prevention, education, and organized community efforts via the Local Oral Health Program managed by the County of Monterey Health Department Public Health Bureau, as well as primary medical care, oral health services, behavioral health services, health education, and other services by the County of Monterey Health Department Clinic Services Bureau to the Monterey County community; and

c. Approve the recommendation of Director of Health Services to accept all non-standard risk provisions in Agreement templates; and

d. Approve and authorize updates to the Mobile Clinic Agreement templates in accordance with local, state, and federal law, subject to review and approval of County Counsel; and

e. Approve and authorize amendments to the Mobile Clinic Agreement templates that do not significantly change the level of risk or the scope of a party’s obligations or responsibilities, subject to review and approval of County Counsel and County Risk Manager; and

f. Approve and authorize the Director of Health Services or their designee to execute the Mobile Clinic Agreement templates with participating School Districts.

RECOMMENDATION:

It is recommended that the County of Monterey Board of Supervisors:

a. Accept and approve a Mobile Clinic Agreement template to be used in partnering with school districts throughout Monterey County for the delivery of primary medical care, oral health services, behavioral health services, health education, and other services by the County of Monterey Health Department Clinic Services Bureau to the Monterey County community; and

b. Approve and accept a second Mobile Clinic Agreement template to be used in partnering with school districts throughout Monterey County for the delivery of oral health prevention, education, and organized community efforts via the Local Oral Health Program managed by the County of Monterey Health Department Public Health Bureau, as well as primary medical care, oral health services, behavioral health services, health education, and other services by the County of Monterey Health Department Clinic Services Bureau to the Monterey County community; and

c. Approve the recommendation of Director of Health Services to accept all non-standard risk provisions in Agreement templates; and

SUMMARY/DISCUSSION:

The Health Department Clinic Services Bureau (Clinic Services) operates twelve community clinic sites, including the Mobile Clinic, designated as Federally Qualified Health Centers (FQHCs) which provide preventive, primary, and specialty medical care services.

On August 29, 2023, Clinic Services was awarded a Health Resources and Services Administration (HRSA) FY 2023 Health Center Program School-Based Service Expansion (SBSE) grant to expand access to primary health care services through new and/or existing delivery sites located at schools via the Mobile Clinic. The Mobile Clinic will provide preventative and primary medical care, oral health services, behavioral health services, health education, and other services to students, their parents/caregivers, and the general community.

Furthermore, the Health Department Public Health Bureau (Public Health) manages a Local Oral Health Program (LOHP) to build capacity and improve the oral health of Monterey County residents through prevention, education, and organized community efforts. The LOHP provides oral health education and preventative dental care services at elementary schools with greater than 80% of students on the Free or Reduced-Price Meals program. Clinic Services and Public Health will partner to implement the LOHP with follow-up oral health screening and oral health services provided by the Mobile Clinic at eligible school sites.

These templates provide for the County of Monterey standards of policy, procedures, and best practices for the delivery of Mobile Clinic services. Using pre-County of Monterey Board of Supervisors approved templates for participating school districts will help streamline administrative processing and reduce the execution timeline, allowing for more timely delivery of services to the Monterey County community. 

School-based service sites are convenient access points for health services for children and adolescents living in underserved communities. Access to school-based health services can positively impact physical and mental health, as well as academic outcomes. Upon successful implementation of school-based service sites, Clinic Services is eligible to apply for ongoing SBSE funding every two years.

No Services will be conducted until a contract is fully executed.

This work supports the County of Monterey Health Department 2018-2024 Strategic Plan Goal(s): 2) Enhance community health and safety through prevention; and 3) Ensure access to culturally and linguistically appropriate, customer-friendly, quality health services. It also supports one or more of the ten essential public health services, specifically:  4) Mobilize community partnerships and action to identify and solve health problems, and 7) Link people to needed personal health services and assure the provision of health care when otherwise unavailable. 

OTHER AGENCY INVOLVEMENT:

The Office of the County Counsel has reviewed and approved the Agreement templates as to legal form. 

FINANCING:

There will be no impact to the General Fund from approval of these Agreement templates. The Agreement templates do not have fiscal provisions as they are funded 100% by the Health Center Program School-Based Service Expansion (SBSE) grant. There are sufficient appropriations in the Health Department’s FY 2024-2025 Adopted Budget (001-4000-HEA007) for the associated costs and revenue and will be included in the Department’s FY 2025-2026 Requested Budget.
