Approve and authorize the Director of Health Services or designee to execute a non-standard Agreement with The Board of Trustees of the University of Illinois to receive training and a license to facilitate the Diabetes Empowerment Education Program (DEEP™), curriculum, for a term commencing upon execution and continuing for five years, for a total Agreement amount of $5,720;
b. Approve the recommendation of Director of Health Services to accept all non-standard risk provisions in Agreement; and
c. Approve and authorize the Director of Health Services or designee to sign up to three future amendments to this Agreement where the total amendments do not exceed 10% ($572) of the original Agreement amount, do not significantly alter the scope of work, and do not exceed the maximum amount of $6,292.

RECOMMENDATION:
It is recommended that the County of Monterey Board of Supervisors:
a. SUMMARY:
This request seeks approval to enter into a five-year licensing agreement with the University of Illinois Chicago to implement DEEP™, an evidence-based, bilingual diabetes self-management curriculum. Diabetes remains a high-priority need in Monterey County. Licensing DEEP™ will allow our Health Department staff, specifically Community Health Workers (CHWs), to deliver a structured, culturally responsive and equity-centered program that addresses gaps in diabetes prevention and management services. Approval supports expansion of evidence-based chronic disease efforts in our community.
DISCUSSION:
Monterey County has a higher diabetes rate (15%) than the California average (12%) and continues to experience significant diabetes-related complications. The 2022 and, most recent, 2025 Monterey County Community Health Needs Assessments also identified diabetes as one of the county’s highest priority areas for action. Local data, including DataShare Monterey County, further confirms the prevalence and impact of diabetes in our community.
DEEP ™ is a diabetes self-management education (DSME) curriculum created through the University of Illinois Chicago. This multicultural, bilingual DSME curriculum was created to help people at-risk or living with diabetes manage their condition and improve their overall quality of life while also reducing the risk of developing diabetic-related complications. It was specifically developed to meet the needs of low-income, racial and ethnic minority populations; however, it has been successfully implemented across diverse populations.
Licensing DEEP™ will allow County of Monterey Health Department to implement a culturally and linguistically appropriate diabetes curriculum that addresses this gap in service. The five-year License Agreement with the University of Illinois will provide our department with the authority to use and reproduce DEEP™ materials, and to train and certify staff and CHWs to deliver and implement the curriculum in our communities. The agreement includes a one-time $600 licensing fee and a $32 fee per workshop delivered. The licensing fee is standard for copyrighted evidence-based curricula and ensures authorized use, program fidelity, and formal certification of staff. Obtaining a license for DEEP™ strengthens the Department’s ability to expand evidence-based chronic disease prevention efforts aligned with community priorities and to improve health outcomes in Monterey County.
This work supports the County of Monterey Health Department 2025-2028 Strategic Plan Goal(s): 1. Build Community Power and Partners’ Capacity to Increase Equity and Improve Health; 2. Provide Exceptional Person-Centered Care through Accessible, Community-Focused Health Services; and 4. Deepen Organizational Effectiveness and Support Long-Term Sustainability. It also supports the following of the ten essential public health services, specifically: 1. Monitor health status to identify and solve community health problems; 2. Diagnose and investigate health problems and health hazards in the community; 3. Inform, educate, and empower people about health issues; 4. Mobilize community partnerships and action to identify and solve health problems; 5. Develop policies and plans that support individual and community health efforts; and 7. Link people to needed personal health services and assure the provision of health care when otherwise unavailable.
OTHER AGENCY INVOLVEMENT:
The Offices of County Counsel, Risk Management, and the Auditor-Controller have reviewed and approved this Agreement as to form, non-standard insurance provisions, and fiscal provisions, respectively.
FINANCING:
There will be no fiscal impact resulting from the approval of this action. Appropriations for this Agreement are included in the Health Department’s (HEA014-8600) Fiscal Year (FY) 2025-26 Adopted Budget. For the FYs out, provisions will be made in the Requested Budget for the appropriate year.

Approval of the DEEP™ licensing agreement advances the Board of Supervisors Strategic Plan Goal of promoting Well-Being and Quality of Life by expanding access to evidence-based, culturally responsive diabetes prevention and management services.
