a. Accept a $14,050 Grant Award from the State of California Department of Public Health (“CDPH”) to provide evidence-based self-management interventions and instructor training to help older adults reduce arthritis-related pain; and
b. Authorize the Director of Health or Assistant Director of Health to execute Agreement Number-16-10263 with the California Department of Public Health for the grant period of July 1, 2016 to June 30, 2017, for the amount not to exceed $14,050; and
c. Authorize the Director of Health or Assistance Director of Health to execute Agreement with Partners in Care Foundation, the self-management program trainer approved by the CDPH for the Grant Award, for the period of September 20, 2016 to September 19, 2017.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Accept a $14,050 Grant Award from the State of California Department of Public Health (“CDPH”) to provide evidence-based self-management interventions and instructor training to help older adults reduce arthritis-related pain; and
b. Authorize the Director of Health or Assistant Director of Health to execute Agreement Number 16-10263 with the CDPH for the grant period of July 1, 2016 to June 30, 2017, for the amount not to exceed $14,050; and
c. Authorize the Director of Health or Assistance Director of Health to execute Agreement with Partners in Care Foundation, the self-management program trainer approved by the CDPH for the Grant Award, for the period of September 20, 2016 to September 19, 2017.
SUMMARY/DISCUSSION:
The Monterey County Health Department provides education that promotes healthy and safe lifestyles and services that prevent or manage chronic disease and injury. On November 22, 2016 Monterey County Health Department received notification as an awardee of the CDPH Grant that will fund travel, training and certification for two Public Health Licensed Vocational Nurses who will provide evidence-based Enhanced Fitness interventions and instructor training to help older adults in Monterey County reduce arthritis pain, increase fitness levels, and lead more active, independent lives.
Partners in Care Foundation is a California nonprofit public benefit corporation that collaborates with physician networks, health plans, community-based organizations, and federal and state agencies to deliver programs and services that protect and support adults with complex health and social service needs, frail elders, people with disabilities, caregivers and families. The Partners in Care Foundation is a Stanford Patient Education Disease Self-Management Education-licensed provider and certified Disease Self-Management Programs trainer. County staff must be certified Disease Self-Management Programs trainers to qualify to enroll in the Stanford arthritis-specific self-management program required to fulfill the scope of work in the CDPH contract. The CDPH contracts with Partners in Care Foundation to provide the training for the County Health Department staff.
Partners in Care Foundation have exceptions to the County standard liability provisions and as such it is the recommendation of the Director of Health or the Assistant Director of Health to receive approval for this Agreement. The start date for the CDPH Agreement is retroactive to July 1, 2016 due to receiving the Agreement late and the start date for the Partners in Care Foundation is retroactive to September 20, 2016 due to the length of time it took to negotiate with the vendor additional terms needed for the Agreement.
The acceptance of this grant will support the Monterey County Health Department 2011-2015 Strategic Plan Initiative: Empower the community to improve health through programs, policies, and activities. Additionally, this work supports two of the 10 Essential Public Health Services: Mobilizing community partnerships and action to identify and solve health problems and inform, educate and empower people about health issues.
OTHER AGENCY INVOLVEMENT:
County Counsel and the Auditor-Controller have reviewed and approved this Agreement as to legal form and fiscal provisions respectively. Risk Management cannot approve as to risk due to the non-standard risk provisions within the Agreement.
FINANCING:
This Agreement is fully funded by the California Department of Public Health. There are sufficient appropriations and revenues included in the Health Department’s Adopted Budget (001-4000-HEA003-8124) to cover Agreement.
