RECOMMENDATION:
It is recommended that the Board of Supervisors:
a.      Reallocate a vacant 1.0 FTE Psychiatric Social Worker II to a 1.0 FTE Public Health Nurse II in Health Department Budget Unit HEA003-8124; and
b.      Authorize the County Administrative Office to incorporate the approved position changes in the FY 2012-13 Adopted Budget.
 
SUMMARY/DISCUSSION:
The Public Health Regional Health Teams (PHRHT) provide targeted case management (TCM) services to County residents, including low-income and Medi-Cal populations, suffering chronic diseases and conditions, and communicable disease investigation and monitoring.  The program is financed with federal and local funds, shared typically at 50% each for eligible program participants.
 
Case management services assist clients to effectively navigate the health care system to access preventative and primary health care services, disease management services, education, social services, and community health programs.  The case manager provides case management and care coordination to referred individuals. In addition, case managers provide chronic disease case management, and targeted health campaigns, which help provide public health safety.
 
Based on current case load and TCM service referral trends, which reflect the community's needs, it was determined that a Public Health Nurse (PHN) is the appropriate staffing classification to provide the case management services required.
 
OTHER AGENCY INVOLVEMENT:
The County Human Resources Department concurs with this recommendation.  Attachment A Budget Information on file with the Clerk of the Board
 
FINANCING:
The additional costs associated with this action are estimated at $4,600 for FY 2012-13. The costs will be financed with salary savings realized from unanticipated vacancies in the Public Health Bureau (PHB).  In subsequent years, the full cost of the reallocated position will be provided for in the PHB's annual operational budget.
