a. Approve and authorize the Director or Assistant Director of the Department of Social Services to execute Amendment #4 to Agreement A-14917 with Central Coast Center for Independent Living to provide long-term support services by adding $39,034 for a revised total contract amount of $382,575; and
b. Authorize the Director or Assistant Director of the Department of Social Services to sign up to three (3) amendments to this Agreement where the total amendments do not exceed ten percent 10% ($38,257) of the revised contract amount and do not significantly change the scope of work.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Approve and authorize the Director or Assistant Director of the Department of Social Services to execute Amendment #4 to Agreement A-14917 with Central Coast Center for Independent Living to provide long-term support services by adding $39,034 for a revised total contract amount of $382,575; and
b. Authorize the Director or Assistant Director of the Department of Social Services to sign up to three (3) amendments to this Agreement where the total amendments do not exceed ten percent 10% ($38,257) of the revised contract amount and do not significantly change the scope of work.
SUMMARY/DISCUSSION:
This amendment adds $39,034 to the existing agreement which are allocated by the California Department of Aging. These funds are to support Aging and Disability Resource Center (ADRC) services in Monterey County that includes enhanced information & assistance; person centered options counseling; short term service coordination; person centered transition support services; and consumer-directed resources for long term support and services (LTSS). Additionally, funding shall promote the implementation of a “no wrong door” approach to provide services in a seamless and easy way.
OTHER AGENCY INVOLVEMENT:
The Auditor Controller and Purchasing have reviewed and approved this Agreement. County Counsel has approved the Agreement as to form.
FINANCING:
This agreement is funded through the California Department of Aging with Aging and Disability Resource Center(ADRC) revenues. There are sufficient appropriations and estimated revenues in the Social Services - Area Agency on Aging’s Budget Unit 001-5010-SOC10-8268 in the Fiscal Year 2022-23 Adopted Budget.

This Agreement correlates to the Health & Human Services Strategic Initiative adopted by the Board of Supervisors by working toward the goals of improving health and quality of life outcomes through the provision of outreach and supportive services to seniors and adults with disabilities.
