RECOMMENDATION:
It is recommended that the Board of Supervisors:
Approve and authorize the Chair of the Board of Supervisors to sign Amendment #3 to Agreement #A-11489 with Central California Alliance for Health for the provision of health plan benefits for In-Home Supportive Service (IHSS) providers adding $2,868,106 and extending the Social Services contract through June 30, 2014, increasing the total contract amount to $7,517,419.
 
SUMMARY/DISCUSSION:
The Central California Alliance for Health has offered the Alliance Care IHSS health plan since July 1, 2005, providing comprehensive and high quality coverage including, hospital, outpatient, primary and specialized care, prescription drugs, mental health services, etc. for a maximum of 488 IHSS providers who meet eligibility criteria.  The Alliance Care IHSS health plan provides culturally and linguistically sensitive access to health care throughout the County.  SEIU 6434, representing IHSS providers, is in support of continuing this health plan for their members. Central California Alliance for Health continues to have all regulatory approvals from the California Department of Managed Health Care and continues to have the necessary Knox-Keene license to provide the Alliance Care IHSS plan.   
 
Pursuant to the Memorandum of Understanding (MOU) approved by your Board on July 31, 2012 between the Monterey County Public Authority for In-Home Supportive Services (IHSS) and SEIU 6434, IHSS providers must work at least 80 hours a month for the last six continuous months to become eligible and once enrolled in the health plan, must work at least 80 hours a month for two of three contiguous months to maintain eligibility.  
 
This Amendment adds additional funding and extends the term of the agreement for one more year.   The maximum number of individuals that can be enrolled is determined by the state funding formula that provides $0.69 of state financial participation for each hour of paid services each month.  Over the three years, health benefits for IHSS Providers were capped at 330 for year one, 475 for year two, and 488 for year three.  Providers who met the initial eligibility criteria were given seniority for enrollment and currently 67 providers are on the waiting list, of which 29 are actively being transferred to open enrollment slots.  
 
OTHER AGENCY INVOLVEMENT:
The Auditor-Controller and Purchasing have reviewed and approved this amendment. County Counsel has approved as to form.
 
FINANCING:
This agreement will be funded approximately 82.5% between Federal/State and IHSS Provider Co-Pays up to the State approved financial participation of $0.60 per hour of paid services, with a County share of 17.5%.  The additional .09 cents per hour authorized above the State approved rate by Monterey County Board of Supervisors has a 50/50 Federal/County share.  The federal financial participation is uncapped.  Sufficient appropriations and estimated revenues are included in the FY 2013-14 Recommended Budget. There is no additional impact to the County General Fund.
