b. Approve and authorize the Director of Health Services or designee to sign the Acknowledgement of Grant Terms and Conditions agreement, required by the CITED Program, in the amount of $2,118,936 for the grant term retroactive to June 1, 2023, through May 31, 2026; and
c. Approve and authorize the Director of Health Services to execute up to three (3) future Amendments to this Agreement that in total do not exceed 10% ($211,893) of the grant Agreement amount, do not significantly change the scope of work, and do not exceed a revised maximum amount of $2,330,829.

RECOMMENDATION:
It is recommended that the County of Monterey Board of Supervisors:
a. SUMMARY/DISCUSSION:
The Monterey County Health Department (MCHD) has been awarded a competitive three-year grant from the DHCS. This grant, offered through the CalAIM PATH CITED Program, is a significant opportunity for the department.
The CalAIM PATH CITED grant program aims to support California's efforts to expand and strengthen its capacity for implementing the CalAIM initiative targeting a wide range of stakeholders, including community-based organizations, public hospitals, county agencies, and others. Importantly, it places particular emphasis on historically under-resourced organizations, which aligns with the goal of advancing health equity and addressing social determinants of health.
One of the primary objectives of the CalAIM PATH CITED grant program is to provide funding for building the necessary capacity and infrastructure to collaborate effectively with managed care plans. This collaboration is vital for the successful implementation of the CalAIM Enhanced Care Management (ECM) and Community Supports (CS) programs, which adopt a holistic, community-based approach to serving individuals with complex health and social needs.
MCHD's successful CalAIM PATH CITED grant application to DHCS has secured $2,118,936.90 in funding over three years, effective from July 1, 2023, through June 30, 2026. These funds will be instrumental in supporting key initiatives, including the establishment of a health information exchange (HIE) among entities responsible for providing ECM and/or Community Supports services. Additionally, the grant will enable the implementation of a closed-loop referral system and enhancements to existing systems to support core monitoring and data reporting requirements for CalAIM.
Furthermore, this funding will facilitate the monitoring of ECM and CS services within Monterey County by allowing the County of Monterey Health Department to employ 2 additional Management Analyst II staff members dedicated to evaluating and monitoring activities. This includes conducting a community health needs assessment to identify gaps in the capacity of ECM and Community Supports service providers and developing an outreach plan for under-resourced and underserved communities.
This Agreement is late in being presented to the Board of Supervisors and is retroactive to June 1, 2023, because the Notice of Award dated October 30, 2023 was received by MCHD on November 7, 2023. Additionally, no funds have been used.
This work supports the Monterey County Health Department 2018-2022 Strategic Plan Goals: 1. Empower the community to improve health through programs, policies, and activities; 2. Enhance community health and safety by emphasizing prevention; and 3. Ensure access to culturally and linguistically appropriate, customer-friendly, quality health services. It also supports one or more of the ten essential public health services, specifically: 3. Inform, educate, and empower people about health issues; 4. Mobilize community partnerships and action to identify and solve health problems; 7. Link people to needed personal health services and assure the provision of health care when otherwise unavailable.
OTHER AGENCY INVOLVEMENT:
The Office of County Counsel and the Auditor-Controller have reviewed and approved the Award and Agreements as to form and fiscal provisions, respectively.
FINANCING:
There is no impact to the General Fund resulting from the approval of this action. Due to the timing of the grant award, the related program revenue and expenditures are not included in the MCHD’s FY 2023-24 Requested Budget (001-4000-8438-HEA014). The Department will evaluate the need to increase appropriations and will bring any necessary actions to the Board for consideration and approval. For FY 2024-25, the anticipated revenue and expenditures will be included in the Department’s Requested Budget.
