a. Accept a grant award of $150,000 (“Grant”) from Central California Alliance for Health (“Alliance”) in support of the development and implementation by the County of Monterey Health Department - Administrative Bureau (“Administrative Bureau”) of the 2025 Community Health Needs Assessment (“CHNA”) and the 2024-2027 Community Health Improvement Plan (“CHIP”) for Monterey County; and
b. Ratify the Director of Health Services’ execution of an Agreement for Medi-Cal Capacity Grant (“Agreement”), Grant number 0624-MCHD-Admin-CHACHIP, with the Alliance, to provide grant funding for the Administrative Bureau’s development and implementation of the 2025 Community Health Needs Assessment (“CHNA”) and the 2024-2027 CHIP, in a Grant amount of $150,000, effective on the last date of the last signature on the Agreement and terminating two years after the Grant effective date or such later date as the parties may mutually agree in writing; and
c. Accept all non-standard provisions in Agreement, as recommended by the Director of Health Services; and
d. Authorize the Director of Health Services or designee to approve up to 3 additional Amendments that do not to exceed 10% ($15,000) of the original Agreement amount, do not significantly alter the scope of services, and do not exceed the maximum amount of $165,000.

RECOMMENDATION:
It is recommended that the County of Monterey Board of Supervisors:
a. Accept a grant award of $150,000 (“Grant”) from Central California Alliance for Health (“Alliance”) in support of the development and implementation by the County of Monterey Health Department - Administrative Bureau (“Administrative Bureau”) of the 2025 Community Health Needs Assessment (“CHNA”) and the 2024-2027 Community Health Improvement Plan (“CHIP”) for Monterey County; and
b. SUMMARY/DISCUSSION:
The Alliance has established Grant #0624-MCHD-Admin-CHACHIP to support the efforts of health care providers and community organizations that advance the Alliance mission to provide timely access to quality health care services, and to increase Medi-Cal capacity in the Alliance’s service area. The Grant focuses on the areas of Access to Care, Healthy Beginnings and Healthy Communities. The Alliance has decided to award funds to the County of Monterey based on a submitted application for funds. The Alliance and County of Monterey agree that all funds awarded as Grant #0624-MCHD-Admin-CHACHIP will support the development of County of Monterey’s 2024-2027 Community Improvement Plan (CHIP) and 2025 Community Health Needs Assessment (CHNA).
The Grant term is September 4, 2024, through December 31, 2025. The Alliance has remitted to the County of Monterey a one-time payment in the Agreement’s full amount of $150,000, based on costs described in the approved Grant Application and payable within twenty (20) business days of the receipt of the signed Agreement.
The agreement’s specific use of funds includes:
a. Subcontracting with Raimi + Associates for the professional design of County of Monterey’s 2024-2027 Community Health Improvement Plan, for ease of public understanding and using the Health Department’s branding guidelines. The subcontract also includes designing and implementing a community distribution plan and creating and mounting Health Department web page elements to promote use of the CHIP. Grant #0624-MCHD-Admin-CHACHIP will also support Health Department participation in community partner meetings to implement joint 2024-2027 CHIP activities and updating the Health Department’s public facing DataShare Monterey County online health outcomes database.
b. Active Health Department staff participation and meaningful contributions to the 2025 CHNA development, throughout the duration of the planning process, as a member of the Monterey County Health Needs Collaborative and providing the Collaborative with a list of key informants to participate in the CHNA data collection process.
This work supports the County of Monterey’s Health Department’s Strategic Plan Goals: 1. Empower the community to improve health. It also supports the following of the ten essential public health services, specifically: 1. Monitor health status to identify and solve community health problems; 3. Inform, educate, and empower people about health issues; 4. Mobilize community partnerships and action to identify and solve health problems.
OTHER AGENCY INVOLVEMENT:
The Office of the County Counsel and Auditor-Controller have reviewed and approved this Grant Agreement as to form and fiscal provisions, respectively.
FINANCING:
There is no impact to the General Fund from approval of this action. The Department will evaluate the need to increase appropriations at mid-year, and if necessary, will bring the appropriate actions before the Board for approval.
