a. Accept the Certificate of Completion for Radiology Equipment Replacement Project for Natividad Medical Center, located at 1441 Constitution Blvd., Salinas, CA 93906, Project No. 9600-12, Bid Package No. NMC-1005; and
b. Authorize the Assistant Administrator of Operations & Support Services to execute and record the Notice of Completion.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Accept the Certificate of Completion for Radiology Equipment Replacement Project for Natividad Medical Center, located at 1441 Constitution Blvd., Salinas, CA 93906, Project No. 9600-12, Bid Package No. NMC-1005; and
b. Authorize the Assistant Administrator of Operations & Support Services to execute and record the Notice of Completion.
SUMMARY/ DISCUSSION:
On December 12, 2012 the Board of Supervisors awarded a construction contract to John F. Otto Inc dba (Otto Construction) to complete the Radiology Equipment Replacement Project for Natividad Medical Center (NMC), Salinas, CA. The Scope of the project was to replace Fluoroscopy unit and two General X-ray units in the Diagnostic Imaging Department located on the first floor of Building 500, and replace a General X-ray unit in the Specialty Clinic located on the second floor of Building 200.
The project started the week of January 14, 2013 and was completed by July 17, 2013. It was determined to have been satisfactorily performed in accordance with the plans and specification.
Otto Construction's contract was in the amount of $479,100.00. There was allowance of $70,000.00 was included within Otto Construction's general contract. One change order was issued and $60,503.31 of the allowance was used, Otto Construction's revised contract amount decreased by $9,496.69 for a new contract amount of $469,603.31. All work was accomplished within the contract amount and on schedule.
The contract is complete in all respects in accordance with the specifications. The work is ready to be accepted by the Board of Supervisors.
OTHER AGENCY INVOLVEMENT:
County Counsel has reviewed and approved this Agreement as to legal form and risk provisions. Auditor-Controller has reviewed and approved this Agreement as to fiscal provisions. The Agreement has also been reviewed and approved by Natividad Medical Center's Board of Trustees
FINANCING:
Radiology Equipment Replacement is fully funded by Capital Budget Contingency ID# B14-2014-X110 for Radiology Rooms (4 Rooms). There were sufficient funds to cover project costs. Final pay applications to Otto Construction in the total amount of $446,123.14 has been processed and a balance of $23,480.17 is due from retention. There is no impact to the General Fund.
