RECOMMENDATION:
It is recommended that the Board of Supervisors:
Approve and authorize the Contracts/Purchasing Officer to execute an Agreement with Rancho Cielo, Inc. and the Monterey County Health Department to provide limited, non-emergent primary medical care services to youth at their campus.  
  
SUMMARY/DISCUSSION:
The Monterey County Health Department's Clinic Services Bureau (Clinic Services) desires to provide limited non-emergent primary medical care services, one day per month, to youth on the Rancho Cielo campus site.  Both Rancho Cielo and Clinic Services will work collaboratively to provide access to health care services to achieve the best health outcomes through delivering a quality of care that is effective, safe, patient-centered, culturally competent, efficient and equitable.
 
Clinic Services and Rancho Cielo share a desire to serve at-risk and underserved populations.  Clinic Services operates seven Federally Qualified Health Center Look-Alike (FQHC) community clinics, committed to serving primarily low-income residents and the medically underserved, many of who are eligible for Medi-Cal, Medicare and other public health care assistance programs.
Rancho Cielo, a 100-acre ranch in Salinas, operates four comprehensive learning and social services programs for underserved youth, ages 16 through 24, from urban City of Salinas and surrounding rural communities within Monterey County.  The vision of Rancho Cielo is to transform the lives of at-risk youth and first-time offenders and empower them to become accountable, competent, productive and responsible citizens through education, vocational training and recreation. Across all four programs, participant income distributions show that 69 percent of the students are in the federal HUD category of very low income- at poverty level.
 
Research in the last decade has shown that students from disadvantaged backgrounds, with access to a school based health center, show improved outcomes in asthma treatment, immunization rates, obesity, family planning and mental health care.
 
A partnership between Clinic Services and Rancho Cielo not only aligns with the County Health Department's strategic plan to work collaboratively in order to achieve the best health outcomes of residents in Monterey County, but it also aligns with the Affordable Care Act- improving quality while controlling costs.
 
OTHER AGENCY INVOLVEMENT:
County Counsel, Contracts/Purchasing and Risk Management have reviewed and concur with this recommendation.  The Agreement is on file with the Clerk of the Board.
 
FINANCING:
There are sufficient appropriations and revenues available in FY 2013-14 Health Department, Clinic Services Bureau's Appropriation 4000- HEA007 Recommended Budget. There is no exchange of funds. Clinic Services will bill Medi-Cal and other insurers to cover services provided to youth.
