RECOMMENDATION:

It is recommended that the Board of Supervisors:

a. Authorize the Director of Health or the Assistant Director of Health to execute Amendment No. 4 to Health Information Exchange Provider Participation Agreement (Monterey County Provider Participants) (“Participation Agreement”) with Central Coast Health Connect, LLC (CCHC), extending the term for one (1) additional year for no-cost participation in the county’s Health Information Exchange (HIE) for the revised term of September 15, 2016 through June 30, 2022; and

b. Authorize the Director of Health or the Assistant Director of Health to sign up to three (3) future amendments to the Participation Agreement where the total amendments do not significantly change the scope of work.

 

SUMMARY/DISCUSSION:

The Health Department Clinic Services Bureau (Health Department) operates ten community clinic sites designated as Federally Qualified Health Centers (FQHCs) which provide preventive, primary, and specialty medical care services.

 

Central Coast Health Connect, LLC (CCHC) is a health information organization (HIO), created to oversee and facilitate a HIE, which will mobilize healthcare information electronically across organizations within the Monterey County. CCHC is managed by a Board of Managers and its founding members include Community Health Innovations, LLC (CHI) and Salinas Valley Memorial Hospital (SVMH).  CCHC has contracted with CHI to perform day-to-day management of the HIE. 

 

The Board of Supervisor’s first approved the Participation Agreement on September 13, 2016 with a term of September 16, 2016 to June 30, 2018. Amendment Nos. 1 to 3 were executed pursuant to the original board order on July 1, 2018, July 1, 2019 and July 1, 2020, respectively, to extend the term on a year-to-year basis as per contract provisions. 


Approval of this request will allows Health Department’s continued participation in the HIE, where clinic staff can
securely access and share patient health data with other participating healthcare providers for the purpose of reducing health disparities,  improving engagement of patients and families, improving care coordination, population and public health and improving communication between hospitals, clinics and care providers while maintaining privacy and security of patient health information. There is no cost to Health Department for this Participation Agreement.

 

This work supports the Monterey County Health Department 2018-2022 Strategic Plan Initiatives: Ensure access to culturally and linguistically appropriate, customer-friendly, quality health services.  It also supports one or more of the ten essential public health services, specifically: 4. Mobilize community partnerships and action to identify and solve health problems, and 7. link people to needed personal health services and assure the provision of health care when otherwise unavailable.

 

OTHER AGENCY INVOLVEMENT:

County Counsel has reviewed as to form.

 

FINANCING:

There are no fiscal provisions included as part of this Amendment.
