Adopt a Resolution to:
a. Accept the Certificate of Completion for Emergency Department Exterior Door Replacement for Natividad Medical Center (NMC), located at 1441 Constitution Blvd., Salinas, CA 93906, Project No. 9600-28, Bid Package No. NMC-1003; and
b. Authorize the NMC Assistant Administrator of Operations & Support Services to execute and record the Notice of Completion.

RECOMMENDATION:
It is recommended that the Board of Supervisors adopt a Resolution:
- Accept the Certificate of Completion for Emergency Department Exterior Door Replacement for Natividad Medical Center (NMC), located at 1441 Constitution Blvd., Salinas, CA 93906, Project No. 9600-28, Bid Package No. NMC-1003; and
- Authorize the NMC Assistant Administrator of Operations & Support Services to execute and record the Notice of Completion.
SUMMARY/DISCUSSION:
On December 11, 2012, the Board of Supervisors awarded a construction contract to Tombleson Inc. to complete the Emergency Department Exterior Door Replacement for NMC, Salinas, CA. The Scope of the project included replacing two automatic entrance doors to provide patient access to the Emergency and Admitting Departments.
The project started the week of April 8, 2013 and was completed by April 15, 2013. It was determined to have been satisfactorily performed in accordance with the plans and specifications. The Tombleson Inc. contract was in the amount of $35,875.00. All work was accomplished within the contract amount and in the number of days scheduled for the project, although the work began later than originally estimated. The contract is complete in all respects in accordance with the specifications. The work is ready to be accepted by the Board of Supervisors.
OTHER AGENCY INVOLVEMENT:
County Counsel has reviewed and approved this Agreement as to legal form and risk provisions. Auditor-Controller has reviewed and approved this Agreement as to fiscal provisions.
FINANCING:
Emergency Department Exterior Door Replacement is fully funded by Capital Budget ID# B-2011-49. Final pay applications to Tombleson Inc. in the total amount of $34,081.25 have been processed and a balance of $1,793.75 is due from retention. There is no impact to the General Fund.
