Approve and authorize the Director of Health Services or designee to execute an Agreement with Salinas Valley Health to plan and implement a technological infrastructure that facilitates care coordination and data exchange, in accordance with California Advancing and Innovating Medi-Cal (CalAIM) requirements via the Monterey County Health Information Exchange (HIE) and Community Information Exchange (CIE) development project, in the amount of $42,925, for a term retroactive to January 1, 2024 through December 31, 2024; and
b. Approve and authorize the Director of Health Services to execute up to three (3) future Amendments to this Agreement that in total do not exceed 10% ($4,292) of the grant Agreement amount, do not significantly change the scope of work, and do not exceed a revised maximum amount of $47,217.

RECOMMENDATION:
It is recommended that the County of Monterey Board of Supervisors:
a. Approve and authorize the Director of Health Services or designee to execute an Agreement with Salinas Valley Health to plan and implement a technological infrastructure that facilitates care coordination and data exchange, in accordance with California Advancing and Innovating Medi-Cal (CalAIM) requirements via the Monterey County Health Information Exchange (HIE) and Community Information Exchange (CIE) development project, in the amount of $42,925, for a term retroactive to January 1, 2024 through December 31, 2024; and
b. Approve and authorize the Director of Health Services to execute up to three (3) future Amendments to this Agreement that in total do not exceed 10% ($4,292) of the grant Agreement amount, do not significantly change the scope of work, and do not exceed a revised maximum amount of $47,217.
SUMMARY/DISCUSSION:
The County of Monterey Health Department (MCHD) has been awarded a competitive three-year grant from the DHCS. This grant, offered through the CalAIM PATH CITED Program, is a significant opportunity for the department.
One of the primary objectives of the CalAIM PATH CITED grant program is to provide funding for building the necessary capacity and infrastructure to collaborate effectively with managed care plans. This collaboration is vital for the successful implementation of the CalAIM Enhanced Care Management (ECM) and Community Supports (CS) programs, which adopt a holistic, community-based approach to serving individuals with complex health and social needs.
MCHD's successful CalAIM PATH CITED grant application to DHCS has secured $2,118,936.90 in funding over three years, effective from July 1, 2023, through June 30, 2026.
The purpose of this Agreement is to develop a framework for cooperation between the County of Monterey Health Department agencies and Salinas Valley Health. MCHD is leading the Monterey County Health Information Exchange (HIE)/ Community Information Exchange (CIE) development project, which will plan and implement a technological infrastructure that facilitates care coordination and data exchange in accordance with CalAIM requirements.
MCHD HIE/CIE development project presents a unique opportunity to further health information exchanges and promote care coordination among various entities responsible for providing CalAIM ECM and CS services. By implementing integrated HIE/CIE systems and establishing a robust interoperable infrastructure, this project will enable seamless sharing of patient data, improve communication, reduce duplication of services, and ultimately enhance the quality of care and services provided to our community members.
MCHD and other partner agencies will collaborate to ensure the effective implementation and integration of this infrastructure. This project is working towards the success of CalAIM's goals, which include promoting whole-person care, addressing social determinants of health, and achieving better health outcomes for vulnerable populations.
These funds will be instrumental in supporting key initiatives, including the establishment of a health information exchange (HIE) among entities responsible for providing ECM and/or Community Supports services.
This work supports the County of Monterey’s Health Department 2018-2024 Strategic Plan Goals: 1. Empower the community to improve health through programs, policies, and activities; 2. Enhance community health and safety by emphasizing prevention; and 3. Ensure access to culturally and linguistically appropriate, customer-friendly, quality health services. It also supports one or more of the ten essential public health services, specifically: 3. Inform, educate, and empower people about health issues; 4. Mobilize community partnerships and action to identify and solve health problems; 7. Link people to needed personal health services and assure the provision of health care when otherwise unavailable.
OTHER AGENCY INVOLVEMENT:
The Office of County Counsel and the Auditor-Controller have reviewed and approved the Award and Agreements as to form and fiscal provisions, respectively.
FINANCING:
There are sufficient appropriations in the Health Department Administration Bureau’s Fiscal Year (FY) 2023-24 Adopted Budget. For FY 2024-25, the anticipated revenue and expenditures will be included in Department’s Requested Budget.
