b. Approve and authorize the Director of Health Services or designee to sign up to three future amendments where the combined amendments do not exceed 10% ($101,764) of the original amount and do not significantly change the scope of services, total not to exceed a maximum of $1,119,405.

RECOMMENDATION:
It is recommended that the County of Monterey Board of Supervisors:
a. Approve and authorize the Director of Health Services or designee to execute Amendment No. 2 with Clear Labs, Inc., for the purchases of a service plan for a Clear Dx System, Clear DX WGS SARS--CoV-2 Reagents, and Clear Dx Microbial Surveillance Reagents and extend the term for an additional two years, for a new term of May 17, 2022 through March 31, 2027, and a total Agreement amount not to exceed $1,017,641; and

SUMMARY/DISCUSSION:
The County of Monterey Health Department is seeking approval to amend its current agreement with Clear Labs, Inc. The original agreement with Clear Labs, Inc., enabled the Public Health Laboratory to purchase SARS-CoV-2 genome sequencing test kits and preventative maintenance service for its Clear Labs instrument. Amendment No.1 allowed the Public Health Laboratory to expand gnomonic surveillance to microbiological specimens like Salmonella, E. coli, Shigella, Listeria and antibiotic resistant bacteria. Amendment No. 2 would allow the Public Health Laboratory to continue SARS-CoV-2 and microbial genomic surveillance by purchasing additional test kits and preventative maintenance service.
Genomic surveillance allows the Health Department to quickly identify disease clusters, identify and track emerging variants, and assess the prevalence, transmissibility and virulence potential of organisms circulating in Monterey County. The Clear Labs instrument provides the Public Health Laboratory with the ability to obtain sequencing results in 24 to 48 hours as compared to several weeks when tested by reference laboratories.
This work supports the County of Monterey Health Department 2018-2024 Strategic Plan Goals: 2. Enhance public health and safety through prevention, 3. Ensure access to culturally and linguistically appropriate, customer-friendly, quality health services, and 4. Engage Health Department workforce and improve operational functions to meet current and developing population health needs. It also supports three of the ten essential public health services, specifically: 1. Assess and monitor population health status, factors that influence health, and community needs and assets; 2. Investigate, diagnose, and address health problems and hazards affecting the population, and 9. Improve and innovate public health functions through ongoing evaluation, research, and continuous quality improvement.
OTHER AGENCY INVOLVEMENT:
County Counsel and Auditor-Controller have reviewed and approved this Agreement as to legal form and fiscal provisions, respectively.
FINANCING:
The amendment is funded 28% by laboratory fee revenue and 72% by Public Health Realignment. Health anticipates utilizing California Department of Public Health Epidemiology and Laboratory Capacity (ELC) grant funds instead of Public Health Realignment as ELC grant funding is available. Appropriations for this amendment are included in the Health Department’s (HEA003-8124) FY 2024-25 Adopted Budget.
