b. Ratify execution by the Director of Health Services of Contract Agreement 24-10200 between the County of Monterey Health Department, on behalf of its Clinic Services Bureau (Clinic Services) and California Department of Public Health (CDPH), for activities for chronic disease improvements to support patients diagnosed or at risk of hypertension and hypercholesterolemia under the California Department of Public Health’s HeartBeatCA Program for an amount up to $199,998 and the program period July 1, 2024 to June 29, 2028.

RECOMMENDATION:

It is recommended that the County of Monterey Board of Supervisors:

a. Accept a Grant Award of up to $199,998 funded by the California Department of Public Health (CDPH) for HeartBeatCA interventions and activities; and

b. Ratify execution by the Director of Health Services of Contract Agreement 24-10200 between the County of Monterey Health Department, on behalf of its Clinic Services Bureau (Clinic Services) and California Department of Public Health (CDPH), for activities for chronic disease improvements to support patients diagnosed or at risk of hypertension and hypercholesterolemia under the California Department of Public Health’s HeartBeatCA Program for an amount up to $199,998 and the program period July 1, 2024 to June 29, 2028.


SUMMARY/DISCUSSION:

California Department of Public Health (CDPH) received an award entitled The National Cardiovascular Health Program by the Centers for Disease Control and Prevention (CDC) for HeartBeatCA interventions and activities. Clinic Services has been allocated a portion of these funds to participate in the HeartBeatCA program. The objective of the HeartBeatCA program is to support public health efforts in chronic disease prevention and management activities to improve the health and quality of life for Californians diagnosed with cardiovascular disease (CFD), hypertension (HTN), and hypercholesterolemia. 

 

By accepting this award, Clinic Services agrees to provide CVD, HTN, and hypercholesterolemia prevention and treatment services to priority populations. These efforts will result in chronic disease improvements to support patients diagnosed or at risk of HTN and hypercholesterolemia using Health Information Technology (HIT) and Electronic Health Records (HER) for referrals to nationally recognized lifestyle modification and chronic disease management programs and social services. Clinic Services will also enhance team-based care approaches and assess quality improvement in systems, policies and best practices to increase knowledge and skills for prevention, and management of HTN, hypercholesterolemia, and CVD.

 

Clinic Services was notified of the grant award on May 8, 2024. 

 

Approval of the recommended actions will allow Clinic Services to comply with CDPH’s requirement to begin grant activities by July 1, 2024.

 

This work supports the County of Monterey Health Department 2018-2024 Strategic Plan Goals: 1.) Engage MCHD workforce and improve operational functions to meet current and developing population health needs. It also supports one or more of the ten essential public health services, specifically: 7.) Link people to needed personal health services and assure the provision of health care when otherwise unavailable, and 8) Assure competent public and personal health care workforce. 

 

OTHER AGENCY INVOLVEMENT:

County Counsel has reviewed as to form. 

 

FINANCING:

The total amount of the CDPH HeartBeatCA grant is $199,998 and its program period is from July 1, 2024 thru June 29, 2028. 

Sufficient appropriations are available in the FY2024-25 Health Department Clinic Services  Requested Budget (4000-HEA007) to implement the CDPH HeartBeatCA grant. There is no financial impact to the General Fund resulting from the approval of this Contract Agreement.
