Approve and authorize the Director of Health or Assistant Director of Health to execute a three-year Mental Health Services Agreement with Front St., Inc. in Santa Cruz, California for the provision of residential services to older adults with serious and persistent mental illness in the amount of $1,395,482 for Fiscal Year (FY) 2018-19, $1,395,482 for FY 2019-20, and $1,450,912 for FY 2020-21, for a maximum County obligation of $4,241,876 for the term of July 1, 2018 through June 30, 2021; and
b. Approve and authorize the Director of Health or Assistant Director of Health to approve up to three (3) future amendments that do not exceed ten percent (10%) ($424,187) of the original Agreement amount and do not significantly alter the scope of services.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Approve and authorize the Director of Health or Assistant Director of Health to execute a three-year Mental Health Services Agreement with Front St., Inc. in Santa Cruz, California for the provision of residential services to older adults with serious and persistent mental illness in the amount of $1,395,482 for Fiscal Year (FY) 2018-19, $1,395,482 for FY 2019-20, and $1,450,912 for FY 2020-21, for a maximum County obligation of $4,241,876 for the term of July 1, 2018 through June 30, 2021; and
b. Approve and authorize the Director of Health or Assistant Director of Health to approve up to three (3) future amendments that do not exceed ten percent (10%) ($424,187) of the original Agreement amount and do not significantly alter the scope of services.
SUMMARY/DISCUSSION:
Front St., Inc. - Drake House is a licensed Residential Care Facility for the Elderly that provides community-based activities for older adults age sixty (60) and over with serious and persistent mental illness with a co-occurring condition. Front St. provides these services at the Drake House facility located in the City of Monterey. The facility is designed to enable seniors to live in an unlocked setting that is designed to provide a maximum level of care and supervision. This forty-nine (49) bed facility was the first of its kind to operate in Monterey County and allows clients previously placed in facilities outside the county to live closer to their families. This ongoing project is a collaborative effort between Monterey and Santa Cruz counties and Front St. This facility addresses one of the unmet service needs of older adults with mental illness as identified in Monterey County’s Proposition 63/Mental Health Services Act (MHSA) community planning process. This Agreement makes twenty-four (24) beds available to Monterey County residents.
This Agreement with Front St., Inc - Drake House is for a three (3) fiscal year term from July 1, 2018 to June 30, 2021, which renews and updates Agreement A-12709 ending June 30, 2018.
This Agreement contains the County’s standard 30-day “no cause” provision (Section IV, B) and an additional defunding provision (Section IV, D), which provides the County the ability to amend or terminate the Agreement in the event of a reduction and/or termination of funding.
This work supports the following Monterey County Health Department 2011-2015 strategic plan initiative: 3) Ensure access to culturally and linguistically appropriate, customer-friendly, quality health services. It also supports one of the ten essential public health services, specifically: 7) Link people to needed personal health services and assure the provisions of health care when otherwise unavailable.
OTHER AGENCY INVOLVEMENT:
County Counsel-Risk Management and Auditor-Controller have reviewed and approved this Agreement as to legal form and fiscal provisions, respectively.
FINANCING:
This Agreement is funded by MHSA and Medi-Cal revenues. The funds for this Agreement ($1,395,482) are included in the Health Department’s Behavioral Health (HEA012, Unit 8410) Fiscal Year 2018-19 Requested Budget. The funds for this Agreement for FY 2019-20 ($1,395,482) and FY 2020-21 ($1,450,912) will be included in the Health Department’s Behavioral Health (HEA012, Unit 8410) Fiscal Year 2019-20 and Fiscal Year 2020-21 Requested Budgets.
