a. Approve and authorize the Contracts/Purchasing Officer to sign Amendment No. 1 to the Agreement with Integra Telecom Holdings, Inc., to provide dedicated telecommunications circuits between the County and the Oregon Community Health Information Network, Inc., to increase the amount by $183,800 for a revised total not to exceed $194,411, and to extend the term to February 28, 2017; and
b. Authorize the Contracts/Purchasing Officer to approve up to three (3) future amendments to this Agreement where the amendments do not significantly change the scope of work or result in an increase to General Fund Contribution.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Approve and authorize the Contracts/Purchasing Officer to sign Amendment No. 1 to the Agreement with Integra Telecom Holdings, Inc., to provide dedicated telecommunications circuits between the County and the Oregon Community Health Information Network, Inc., to increase the amount by $183,800 for a revised total not to exceed $194,411, and to extend the term to February 28, 2017; and
b. Authorize the Contracts/Purchasing Officer to approve up to three (3) future amendments to this Agreement where the amendments do not significantly change the scope of work or result in an increase to General Fund Contribution.
SUMMARY/DISCUSSION:
The Monterey County Health Department, Clinic Services Bureau ("Clinic Services") entered into a three year agreement with Integra Telecom Holdings, Inc., beginning August 1, 2012 to July 31, 2015, in the amount not to exceed $45,000. The agreement is to support mission-critical telecommunication circuits between the County and the Oregon Community Health Information Network, Inc. (OCHIN), by providing Clinic Services 3Mbps of bandwidth to support clinic management functions such as email, web browsing and simultaneous use of Electronic Medical Records and real-time image downloads.
Clinic Services, which operates seven Federally Qualified Health Center-Look Alike clinic sites, one satellite clinic and two future satellite clinic sites, is requesting an increase of bandwidth of 17Mbps, for a total of 20Mbps, in order to support the addition of providers, equipment, clinic sites and all clinic management functions. The additional bandwidth will also allow providers at Natividad Medical Center, "view only" access to Clinic Services' patient records, creating higher quality and more efficiency through healthcare technology integration.
Integra Telecom Holdings, Inc. is the only directly connected provider to the OCHIN network. In addition, the cost to the Health Department is lower than rates for similar telecommunications due to a bulk purchase price provided to OCHIN by Integra.
OTHER AGENCY INVOLVEMENT:
The Office of the County Counsel, Information Technology Department, and Contracts/Purchasing has reviewed and concur with this recommendation. The current circuit will be terminated in the Information Technology Department (ITD) data center on Moffett Street. The Agreement and Amendment No. 1 are on file with the Clerk of the Board.
FINANCING:
This agreement is funded by the Clinic Services Bureau operating budget. There are sufficient appropriations in Fiscal Year 2013-14 Health Department Budget 4000-HEA007.
