RECOMMENDATION:
It is recommended that the County of Monterey Board of Supervisors:
a. Approve and authorize the Contracts/Purchasing Officer or designee to execute Amendment No.1 to Agreement No. A-17143 with the Monterey County Office of Education, for the provision to establish the infrastructure needed to develop an Enhanced Care Management (ECM) Hub, adding $4,500 for a new total Agreement amount of $421,293 with no change to the Agreement term of July 1, 2024 through June 30, 2025; and

SUMMARY/DISCUSSION:
The County of Monterey Health Department has been awarded $1,262,058 in funding from the California Advancing and Innovating Medi-Cal (CalAIM) California Providing Access and Transforming Health (PATH) Capacity and Infrastructure, Transition, Expansion and Development (CITED) Round 3 program, through the Department of Health Care Services (DHCS).
This Amendment to the existing agreement with the Monterey County Office of Education (MCOE) updates both the funding allocation and Scope of Work (SOW). Specifically, it reallocates $421,293 of the awarded funds to support the expansion of Enhanced Care Management (ECM) services through the development of an ECM Hub. The Hub will focus on serving children and families experiencing homelessness, children and youth involved in Child Welfare, non-English speakers, and youth involved in the juvenile justice system. The reallocated funding will be used to build the necessary infrastructure for the ECM Hub, including hiring dedicated staff and establishing operational processes to support service delivery.
The decision to expand these services was driven by a gap analysis conducted by the Central California Alliance for Health (CCAH), which highlighted the need for more comprehensive support for these groups. With this grant, MCOE plans to provide a range of wraparound services, including housing assistance, tenancy and sustainability services, referrals to community support, and outreach and engagement initiatives.
This work supports the County of Monterey Health Department 2025-2028 Strategic Plan Goals; 1. Empower the community to improve health, 3. Ensure access to culturally and linguistically appropriate, customer-friendly, quality health services. It also supports the following of the ten essential public health services, specifically 7. Link people to needed personal health services and assure the provision of health care when otherwise unavailable.
OTHER AGENCY INVOLVEMENT:
The Offices of County Counsel and Auditor-Controller have reviewed and approved this Agreement as to legal form and fiscal provisions, respectively.
FINANCING:
There is no impact to the General Fund from approval of this action. This Agreement is financed with funds from the PATH CITED grant. There are sufficient appropriations in the Health Department Administration Bureau’s (001-4000-8438-HEA014) Fiscal Year 2024-25 Adopted Budget.
