RECOMMENDATION:
It is recommended that the Board of Supervisors adopt a Resolution:
 
a. Accepting the Certificate of Completion for construction of the Integrated Health Clinic Services Center, 299 12th Street, Marina CA, Project No. 8834, Bid Package No. 10351, performed by John F. Otto, Inc. dba Otto Construction; and
 
b. Authorizing the Director of Public Works to execute and record the Notice of Completion for the Integrated Health Clinic Services Center, 299 12th Street, Marina CA, Project No. 8834, Bid Package No. 10351, performed by John F. Otto, Inc. dba Otto Construction.
 
SUMMARY/DISCUSSION:
On May 22, 2012, the Board of Supervisors awarded a contract to John F. Otto, Inc., dba Otto Construction for construction improvements of the Integrated Health Clinic Services Center, 299 12th Street, Marina, CA, Project No. 8834, Bid Package No. 10351 (Project), in the amount of $2,576,050, with an authorized contingency of $257,605.  The Project consisted of improvements to the site and building for the purpose of providing homeless assistance and various outreach services to the County.  Construction began in June 2012, and by November 2012, the continuing discovery of previously unknown issues resulted in additional funding requirements.  On January 29, 2013, the Board of Supervisors authorized a construction contingency increase of $1,077,436 to cover the large change orders to provide for mitigating mold removal; rebuilding all exterior walls; removing and replacing the mechanical ducting system; and rerouting domestic and fire lines and associated backflow preventers.  The contract is now complete in all respects in accordance with the Plans and Special Provisions.  The work is ready to be accepted by the Board.
 
OTHER AGENCY INVOLVEMENT:
The Resource Management Agency-Public Works staff worked closely with the Health Department Executives and Health Department Finance in determining the required course of action for the Project.
 
FINANCING:
There is no financial impact to the General Fund.  The project was fully funded by the Health Department, Behavioral Health Unit HEA002.  Funding was derived from the Mental Health Services Act Proposition 63, Capital Facilities and Technological Funds (MHSA), and unexpended Mental Health monies.  Total project cost, including management, design, and construction, is $4,513,402.
