SUMMARY/DISCUSSION
The Department of Health Care Services (DHCS) received federal approval under its California Advancing and Innovating Medi-Cal (CalAIM) 1115 Demonstration for Providing Access and Transforming Health (PATH) capacity building funds to support the Justice-Involved Reentry Initiative. Under the CalAIM 1115 Demonstration, DHCS received approval to disseminate up to $410 million in capacity building funds to support the planning and implementation of pre-release and reentry services in the 90 days prior to an individual’s release into the community. California is the first state in the nation to receive federal approval to provide pre-release services.
The PATH Justice-Involved Reentry Initiative capacity building program will provide funding to support the planning and implementation of the provision of targeted pre-release Medi-Cal services to individuals in state prisons, county jails, and youth correctional facilities who meet the eligibility criteria. This initiative aims to advance health equity and improve physical and behavioral health outcomes for the justice-involved population to avert unnecessary use of inpatient hospitals, psychiatric hospitals, nursing homes, emergency departments, and other forms of costly and inefficient care.
Behavioral health links seek to ensure continuity of treatment for individuals who receive behavioral health services while they were incarcerated and who wish to continue to receive these services from SMHS, DMC, and/or DMC-ODS in the community. Behavioral health links are also for individuals who receive medication treatment for Substance Use Disorder (SUD), including through the MCP provider network. CFs and County Behavioral Health Agencies will work in partnership to facilitate professional-to-professional clinical handoffs to post-release providers and share information with the member’s health plan (e.g., county MHP, DMC/DMC-ODS counties, and MCPs as needed) or the provider who will prescribe the medication for substance use disorder.
OTHER AGENCY INVOLVEMENT
Staff has consulted with the Office of County Counsel. Risk Management reviewed this agreement due to non-standard insurance.

There are enough appropriations in the Health Department's Behavioral Health Bureau (Fund 023, Unit 8410, Appr HEA012) FY 2023-24 Adopted Budget to absorb the costs of this grant and no increase in appropriations is required.
