RECOMMENDATION:
It is recommended that the County of Monterey Board of Supervisors:
a. Approve and authorize the Contracts/Purchasing Officer or designee to execute a non-standard Agreement with the Monterey County Office of Education, for the provision to establish the infrastructure needed to develop an Enhanced Care Management (ECM) Hub, for a retroactive term of July 1, 2024 through June 30, 2025, for a total Agreement amount of $419,793; and
b. Approve the recommendation of the Director of Health Services to accept non-standard insurance provisions in Agreement.
SUMMARY/DISCUSSION:
The County of Monterey Health Department has been awarded $1,262,058 in funding from the California Advancing and Innovating Medi-Cal (CalAIM) California Providing Access and Transforming Health (PATH) Capacity and Infrastructure, Transition, Expansion and Development (CITED) Round 3 program, through the Department of Health Care Services (DHCS). Health seeks to allocate a portion of this funding, $419,793, to the Monterey County Office of Education (MCOE) to expand Enhanced Care Management (ECM) services by establishing an ECM Hub that is focused on serving children and families experiencing homelessness, children and youth involved in Child Welfare, non-English speakers, and youth involved in the juvenile justice system. Specifically, funding will be used to establish the infrastructure needed, including hiring dedicated staff and establishing the processes necessary, to develop an ECM Hub.
The decision to expand these services was driven by a gap analysis conducted by the Central California Alliance for Health (CCAH), which highlighted the need for more comprehensive support for these groups. With this grant, MCOE plans to provide a range of wraparound services, including housing assistance, tenancy and sustainability services, referrals to community support, and outreach and engagement initiatives. This Agreement is retroactive due to the timing of the grant award, which was July/August.
This Agreement is late in being presented to the Board of Supervisors and is retroactive to July 1, 2024, due to lengthy negotiations over the insurance requirements.
This work supports the County of Monterey’s Health Department’s 2018-2024 Strategic Plan Goals; 1. Empower the community to improve health, 3. Ensure access to culturally and linguistically appropriate, customer-friendly, quality health services. It also supports the following of the ten essential public health services, specifically 7. Link people to needed personal health services and assure the provision of health care when otherwise unavailable.
OTHER AGENCY INVOLVEMENT:
The Offices of County Counsel and the Auditor-Controller have reviewed and approved this Agreement as to form and fiscal provisions, respectively.
FINANCING:
There is no impact on the General Fund from approval of this action. This Agreement is financed with funds from the PATH CITED grant. Due to the timing of the grant award, the associated appropriations are not included in the Health Department Administration Bureau’s FY 2024-25 Adopted Budget (001-4000-8438-HEA014). The Bureau will assess the need to increase appropriations at mid-year, and if necessary, will bring the needed actions before the Budget Committee and full Board for approval.
