Title: a. RECOMMENDATION:

It is recommended that the County of Monterey Board of Supervisors:

a. Approve and authorize the Contracts/Purchasing Officer or designee to execute a non-standard Agreement with Monterey County Children and Families DBA First 5 Monterey County, to assist in the establishment of infrastructure needed to develop an Enhanced Care Management (ECM) Hub focused on the early childhood population in Monterey County, for a retroactive term of July 1, 2024 through June 30, 2025, for a total Agreement amount of $302,351; and

b. Approve the recommendation of Director of Health Services to accept all non-standard provisions in Agreement, including indemnification.

SUMMARY/DISCUSSION:

The County of Monterey Health Department has been awarded $1,262,058 in funding from the California Advancing and Innovating Medi-Cal (CalAIM) California Providing Access and Transforming Health (PATH) Capacity and Infrastructure, Transition, Expansion and Development (CITED) Round 3 program, through the Department of Health Care Services (DHCS). A portion of this funding, $302,351, is allocated to the Monterey County Children and Families Commission DBA First 5 Monterey County (First 5) to expand Enhanced Care Management (ECM) services focused on the early childhood population in Monterey County.

 

This work supports the County of Monterey’s Health Department’s 2018-2024 Strategic Plan Goals: 1. Empower the community to improve health, 3. Ensure access to culturally and linguistically appropriate, customer-friendly, quality health services. It also supports the following of the ten essential public health services, specifically 7. Link people to needed personal health services and assure the provision of health care when otherwise unavailable.

 

OTHER AGENCY INVOLVEMENT:

The Offices of County Counsel and the Auditor-Controller have reviewed and approved this Agreement as to form and fiscal provisions, respectively.

 

FINANCING:

This agreement is funded by California Department of Health Care Services, California Advancing and Innovating Med-Cal (CalAIM) Providing access and Transforming Health (PATH) Round 3. There are sufficient appropriations in the Health Department Administration Bureau’s (8600-HEA014) Fiscal Year (FY) 2024-25 Adopted Budget.
