a. Approve and authorize the Director or Assistant Director of the Department of Social Services to sign an agreement with Health Projects Center for $261,311, to provide Caregiver Support and Fall Prevention services to Monterey County individuals with disabilities, older adults and their care givers for the period of July 1, 2022 through June 30, 2023; and
b. Authorize the Director or Assistant Director of the Department of Social Services to sign up to three (3) amendments to the agreement, where the total amendments do not exceed 10% ($26,131) of the contract amount and do not significantly change the scope of work.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Approve and authorize the Director or Assistant Director of the Department of Social Services to sign an agreement with Health Projects Center for $261,311, to provide Caregiver Support and Fall Prevention services to Monterey County individuals with disabilities, older adults and their care givers for the period of July 1, 2022 through June 30, 2023; and
b. Authorize the Director or Assistant Director of the Department of Social Services to sign up to three (3) amendments to the agreement, where the total amendments do not exceed 10% ($26,131) of the contract amount and do not significantly change the scope of work.
SUMMARY/DISCUSSION:
This agreement provides the following Family Caregiver Support and Fall Prevention services to older adults and individuals with disabilities in Monterey County:
- Caregiver counseling, which may range from guidance with caregiving responsibilities to therapy for stress, depression, and loss;
- Community education on caregiving designed to educate groups of current or potential caregivers and provide them with caregiver support resources and services;
- Respite Care services that include the provision of assistance to the care receiver with eating, bathing, toileting, transferring, and or dressing by an appropriately skilled provider; and
- Fall Prevention services, including participant enrollment, home assessment, consultation, referrals, client satisfaction surveys, and coordination of building contractor services for income eligible individuals with disabilities or persons 60 years of age or older at risk of falling or being institutionalization.
OTHER AGENCY INVOLVEMENT:
The Auditor-Controller and Purchasing have reviewed and approved this Agreement. County Counsel has approved the Agreement as to form.
FINANCING:
Funding is provided by the Older Americans Act of $72,680 and State Fall Prevention Grant funds of $188,631. Sufficient appropriations and estimated revenues in the Area Agency on Aging 001-5010-SOC010 are included in the FY2022-23 Recommended Budget.
BOARD OF SUPERVISORS STRATEGIC INITIATIVES:
This Agreement correlates to the Health & Human Services Strategic Initiative adopted by the Board of Supervisors by working towards the goals of improving health and quality of life outcomes by providing caregiver support and fall prevention services for seniors and individuals with disabilities and their caregivers in Monterey County.
