RECOMMENDATION:

It is recommended that the Board of Supervisors:

Approve and authorize the Director of Health Services or designee to execute a Professional Services Agreement with Allen Radner, MD, a physician who is qualified and licensed to practice medicine or other health care profession in the State of California, to render and perform medical or other health care services under the direction and designation of County, on an “at-will” basis, as the Tuberculosis Controller, in the Public Health Bureau, for the term of October 1, 2026, through September 30, 2031, for a total amount not to exceed $520,780; and

SUMMARY:
Allen Radner, MD, has served as the County of Monterey TB Controller since July 2016. The Public Health Bureau seeks to continue its existing arrangement with Dr. Radner for critical TB prevention and control services.

DISCUSSION:
The Health Department Public Health Bureau’s Tuberculosis Control Unit provides leadership and resources to prevent and control tuberculosis (TB) within Monterey County. Each local health jurisdiction in California engages the services of a licensed physician with experience in infectious diseases to act as the jurisdiction’s TB Controller. The TB Controller provides expert consultation to the medical community on the appropriate protocols and care of persons diagnosed with or suspected to have active TB and latent TB infection. The TB Controller also provides guidance to local health department staff on infection control, testing and treatment protocols, outbreak management, and case management services.

This work supports the County of Monterey Health Department 2025-2028 Strategic Plan Goals: 1. Build community power and partners’ capacity to increase equity and improve health; and 2. Provide exceptional person-created care through accessible, community focused health services. It also supports five of the ten essential public health services, specifically: 1. Assess and monitor population health status, factors that influence health, and community needs and assets, 2. Investigate, diagnose, and address health problems and hazards affecting the population, 3. Communicate effectively to inform and educate people about health, factors that influence it, and how to improve it, 7. Assure an effective system that enables equitable access to the individual services and care needed to be healthy, and 8. Build and support a diverse and skilled public health workforce.

OTHER AGENCY INVOLVEMENT/COMMITTEE ACTIONS:
The Offices of the County Counsel and Auditor-Controller have reviewed and approved this agreement as to legal form and fiscal provisions, respectively.

FINANCING:
This agreement is 100% funded through Health Realignment. There are sufficient appropriations in the Health Department’s (001-4000-8124-HEA003) FY 2026-27 Adopted Budget.

BOARD OF SUPERVISORS STRATEGIC PLAN GOALS:
This Agreement advances the Board of Supervisors' Strategic Plan Goal A: Well-being and Quality of Life, specifically the key objectives of Health and Facilities (provide exceptional facilities and health programs to support quality of life for residents) and Services (provide service enhancements to support well-being and quality of life).  Early detection of TB disease, combined with access to appropriate treatment and supportive services, is essential to reduce the spread of TB within Monterey County communities. 

This Agreement provides local healthcare providers and the Public Health Bureau with access to the infectious diseases expertise necessary to provide high-quality TB case management services to Monterey County residents.
