RECOMMENDATION:

It is recommended that the Board of Supervisors:

a. Authorize the Contracts and Purchasing Officer or designee to execute a contract with Correctional Healthcare Partners Monterey, Inc. (CHP Monterey, INC.), effective January 1, 2026, through June 30, 2031, for the medical, dental, psychiatric, and Jail Based Competency Treatment services for incarcerated persons at the Monterey County Jail in an amount not to exceed $139,396,760; and  

b. Authorize the Contracts and Purchasing Officer or designee to sign up to two (2) future one (1) year amendments to this agreement where the total amendment(s) do not significantly change the scope of work and do not cause an annual increase per amendment of more than the July 1, 2030 through June 30, 2031annual contract amount plus ten percent 10%; or

c. SUMMARY/DISCUSSION:

In February 2025, the Sheriff’s Office, in collaboration with the Contracts and Purchasing Office issued a Request For Proposal (RFP) #10939 for jail medical, dental, psychiatric, and Jail Based Competency Treatment services. 

 

The Sheriff’s Office engaged with the Contracts and Purchasing Office, Natividad Medical Center, Monterey County Health Services, and the Los Angeles County Department of Health Services to participate in the evaluation and selection process.  This broad approach to evaluating the proposals provided the Sheriff’s Office and County a strong sense of transparency and subject matter expertise that increased the breadth and depth of inquiry in the process.

 

Final proposals were due in April 2025.  In June 2025, the RFP review and evaluation process was completed and CHP Monterey, INC. was selected as the future medical provider beginning January 1, 2026 for those in the custody and care of the Monterey County Sheriff’s Office at the Monterey County Jail.  The contract will be effective January 1, 2026, through June 30, 2031, and includes provisions for possible contract extensions.  The establishment of this contract will see a significant increase in cost for Correctional Healthcare Services and staffing for the delivery of correctional healthcare services at the Monterey County Jail.  Correctional healthcare contracted staffing will increase from current staffing of 54 FTE to 89.95 FTE which directly impacts the delivery of healthcare services.

 

Medical and mental health care at the state and county levels in California has undergone a significant transformation in the last several years.  These changes have been driven by both advocacy and litigation. Locally, these changes have been further amplified by Hernandez et al v. County of Monterey, et. al., Case Number: 5:13-cv-02354-BLF settlement agreement and the steady increase in vulnerable population incarcerated persons.  We also recognize the need for increased medical and mental health services within this segment of our community.  Importantly, community conversations, advocacy and the CalAIM process have afforded us an opportunity to unify in meaningful ways community care and correctional care.  This has resulted in the Monterey County Sheriff’s Office Corrections Operations Bureau developing and expanding our medical and mental health programs to offer coordinated in-custody care and seamless transitions between the community and custody.

 

These changes have also created a deeper clinical approach to health assessments, and as a result, the jail is experiencing a substantial increase in observed and reported illness and injury during the intake process.  In turn, this is creating a higher demand for follow up treatment during their custody stay.  Additionally, since implementation of the 2011 Public Safety Realignment (AB 109) the Sheriff’s Offices has realized longer custody stays among the incarcerated population and increased chronic medical care issues. 

 

This contract incorporates performance metrics to ensure accountability and contract adherence.  These performance metrics are mirrored to the NCCHC standards for medical and mental health services in jails.  NCCHC is the leader in correctional healthcare regulation and accreditation.  NCCHC regularly works with the United States Bureau of Justice Assistance to advance Federal medical and mental health programs throughout the nation and are strong advocates for whole person care both in custody and when returning to the community.

 

CHP Monterey, INC. has a proven ability to maintain a high level of stable staffing.  Their experience in the California correctional medical services market has given them the ability to recruit and retain some of the most highly-trained medical professionals in the State of California.  CHP Monterey, INC. has realized their market influence and is working to increase collaboration and partnerships with local colleges and universities to allow nursing and mental health students the ability to receive training and introduction into correctional healthcare.

 

OTHER AGENCY INVOLVEMENT/COMMITTEE ACTIONS:

The Office of County Counsel has reviewed and approved this amendment as to form. Staff from the Health Department, Contracts and Purchasing Office, County Counsel, Risk Management, and Natividad Medical Center were engaged throughout this agreement’s RFP process. 

 

FINANCING:

The Sheriff’s Office FY25-26 budget includes $15.1 million for Correctional Healthcare Services.  Total estimated cost for FY25-26 when combining the services of our current vendor and CHP Monterey, Inc. is $19.3 million.  The Sheriff’s Office FY25-26 budget will require approximately $4.2 million in augmentation funding to fund Correctional Healthcare Services this fiscal year.  The Sheriff's Office will work with the CAO’s Office to identify funding for this fiscal year and return to the Board of Supervisors after the County FY25-26 mid-year budget presentation.  Funding for future years of this agreement will be addressed during the annual County budget building process.
