RECOMMENDATION:

 

It is recommended the Board of Supervisors:

 

Authorize the Chief Executive Office for Natividad Medical Center (NMC) or his designee to execute renewal and amendment No. 4 to the agreement with Neurologic, LLC,  for Intraoperative Neurophysiological Monitoring Services, to extend the agreement for an additional one (1) year period (retroactive to January 1, 2022 through December 31, 2022) for a revised full agreement term of October 1, 2015 through December 2022, and to add $100,000 for a revised total agreement amount not to exceed amount of $406,000. 

 

SUMMARY/DISCUSSION:

 

Neurologic LLC provides 24 hours per day, 7 day a week, intraoperative neurophysiological monitoring (IOM) on an as needed basis to patients undergoing Orthopedic, ENT, Vascular, Neurosurgical and spinal procedures.  IOM allows technicians to observe in real time a patient’s neurophysiology during a surgical procedure.  The monitoring provides real time information to the surgeon and staff as to the integrity of the patient’s nervous system during surgery.  The technicians apply electrodes (surface and subdermal) on the patient to achieve appropriate signals.  Once the signals are received, the technician monitors the patient’s nervous system via computer during the procedure.  This provides the surgeon valuable information regarding the integrity of the nervous system as they proceed through the case. 

 

Multiple methods are available in determining and detecting changes in the nervous tissue that is at risk for injury during procedures including:

                     Sensory and motor evoked potential (EP)

                     Electromyography (EMG)

                     Compound Muscle action potentials

                     Electroencephalography (EEG)

 

Oversight is provided with the transfer of live data through HIPAA compliant connections during the procedure. These services are beneficial to NMC because of the types of surgery being performed. The neurosurgeons expect this type of support while performing these delicate surgeries to reduce the risk of injury to the patient.  The benefit to patients is the same as the risk of injury to their nervous system is reduced which allows them to return to their normal function. The deliverables in this instance should be relatively simple to monitor.  They include the availability of the Neurologic staff when needed and the availability of reports regarding the competency of their staff, the written report of the procedure for the patient’s record, and any other report requested by NMC.  

 

NMC is requesting that the start date of this amendment be retroactive to January 1, 2022.  The hospital had intended to bring in a new neurosurgical system so this one lapsed, however, plans have now changed and the hospital would like to continue using the services of Neurologic, LLC.

 

OTHER AGENCY INVOLVEMENT:

 

County Counsel has reviewed and approved this renewal and amendment No. 4 as to legal form and risk provisions, and the Auditor-Controller has reviewed and approved as to payment provisions. The renewal and amendment No. 4 have also been reviewed and approved by NMC’s Finance Committee and by its Board of Trustees on March 11, 2022.

 

FINANCING:

 

The cost for this renewal and amendment No. 4 is $100,000 of which $60,000 is included in the Fiscal Year 2021-22 Adopted Budget. Amounts for remaining years of the Agreement will be included in those budgets as appropriate.
