Approve and authorize the Contracts/Purchasing Officer or designee to execute Amendment No. 3 to Agreement No. A-15021, pursuant to RFP #10742, with Firato Service Company, Inc., for specialized custodial maintenance services, to extend the term of the contract for one (1) additional year, for a revised full term of January 1, 2021 to December 31, 2024, and increase the contract amount by $450,000, for a revised contract amount not to exceed $1,547,749, and
b. Authorize the Contracts/Purchasing Officer or designee to sign up to three (3) future amendments to this Agreement where the amendments in total do not exceed 10% of the contract amount ($15,477) and do not significantly change the scope of work.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Approve and authorize the Contracts/Purchasing Officer or designee to execute Amendment No. 3 to Agreement No. A-15021, pursuant to RFP #10742, with Firato Service Company, Inc., for specialized custodial maintenance services, to extend the term of the contract for one (1) additional year, for a revised full term of January 1, 2021 to December 31, 2024, and increase the contract amount by $450,000, for a revised contract amount not to exceed $1,547,749, and
b. Authorize the Contracts/Purchasing Officer or designee to sign up to three (3) future amendments to this Agreement where the amendments in total do not exceed 10% of the contract amount ($15,477) and do not significantly change the scope of work.
SUMMARY/DISCUSSION:
The Health Department Clinic Services Bureau (Clinic Services) operates ten community clinic sites designated as Federally Qualified Health Centers (FQHCs) which provide preventive, primary, and specialty medical care services.
On April 2, 2020, Clinic Services published RFP #10742 to solicit one or more new agreements for specialized custodial maintenance services from qualified contractors and Firato Service Company, Inc., was selected as the most qualified vendor based on the bid selection process. Service areas will include exam rooms, waiting areas, offices, common areas, halls, bathrooms, multi-purpose rooms and a playground, as set forth in the terms and conditions of RFP# 10742 as well as in the new Agreement.
Amendment No. 1 was executed on January 1, 2021 to incorporate additional scope documents and Amendment No. 2 was approved on October 18, 2021 to add an additional site. Approval of the recommended action will maintain uninterrupted specialized custodial maintenance services in place for all Clinics Services locations while also supporting incremental costs associated with future servicing resulting from capital expansion projects, such as the Alisal Integrated Health Center project, with an anticipated construction completion of Fall 2023.
This work supports the Monterey County Health Department 2018-2022 Strategic Plan initiatives: 2) Enhance community health and safety through prevention. It also supports one of the ten essential public health services, specifically: 6) Enforce laws and regulations that protect health and ensure safety.
OTHER AGENCY INVOLVEMENT:
The Office of County Counsel has approved as to form. The Office of the Auditor-Controller has approved as to fiscal provisions.
FINANCING:
Sufficient funds are available in the FY 2023-2024 Adopted Budget for Clinic Services Bureau, Health Department (4000-HEA007). There is no financial impact to the General Fund resulting from approval of this Agreement. Services will be funded by direct revenues, such as payments from Medi-Cal and Medicare.
