a. Ratify acceptance of the $25,000 Medi-Cal Member Outreach and Retention (MOR) Tier 1 grant award from Central California Alliance for Health (CCAH);
b. Ratify execution by the Director of Health Services of the Medi-Cal Capacity Grant Program Agreement with CCAH, effective upon full execution by both parties and expiring 14 months thereafter, for the MOR Tier 1 grant program in the amount of $25,000;
c. Accept all non-standard provisions in Grant Agreement, as recommended by the Director of Health Services;
d. Approve and authorize the Director of Health Services or designee, to execute any and all ancillary, related, or correlating documents necessary to effectuate the execution and administration of Grant Agreement, including non-substantive amendments, assignments, and related instruments consistent with the terms approved by the Board; and
e. Authorize the Director of Health Services or designee to approve up to three additional Amendments that do not significantly alter the scope of services.

RECOMMENDATION:
It is recommended that the County of Monterey Board of Supervisors:

SUMMARY:
County of Monterey Health Department (MCHD) has provided CHW services over the past decade through a combination of grant and county funding. Central California Alliance approved MCHD Office of Equity Division Medi-Cal Member Outreach and Retention Tier 1 grant application in the amount of $25,000 on April 24, 2026. The grant term is 14 months.
MCHD intends to accept the award to support efforts that advance timely access to quality health care services and to increase Medi-Cal capacity in Monterey County. Specifically, to support community-based efforts to keep Alliance Medi-Cal members enrolled in Medi-Cal by funding linguistically and culturally responsive outreach and education activities, as described in the approved Grant Application.
DISCUSSION:
MCHD applied to the CCAH MOR grant for a maximum amount of $25,000 for a 14-month grant period. The MOR grant program supports community-based efforts to keep Alliance Medi-Cal members enrolled in Medi-Cal by funding outreach, education, and Medi-Cal renewal assistance in the Alliance service areas.
The funding will be used to increase Medi-Cal member awareness about requirements to maintain Medi-Cal coverage in Monterey County by supporting outreach and Medi-Cal workshop efforts. The MCHD, Office of Equity, will oversee the grant activities and deliverables. Specifically, funds will be used for Community Service Aide IIIs, who serve as CHWs covering salary expenses for outreach efforts aimed at disseminating Medi-Cal policy changes and benefits in Monterey County.
Due to application and execution deadlines established by Central California Alliance for Health (CCAH), the Director of Health Services was required to execute the grant agreement on an expedited basis to ensure MCHD’s eligibility to receive and secure the funding. Therefore, this item is being presented to the Board of Supervisors for ratification of the Director’s prior acceptance of the grant award and execution of the agreement.
This work supports the Monterey County Health Department 2025-2028 Strategic Plan Strategic Plan Initiative: Goal 2. Provide Exceptional Person-Centered Care through Accessible, Community-Focused Health Services. It also supports 3 of the ten essential public health services, specifically: 3. Communicate effectively to inform and educate people about health, factors that influence it, and how to improve it; 4. Strengthen, support, and mobilize communities and partnerships to improve health; 7. Assure an effective system that enables equitable access to the individual services and care needed to be healthy
OTHER AGENCY INVOLVEMENT:
The Offices of County Counsel and the Auditor-Controller have reviewed and approved this Agreement as to form and fiscal provisions, respectively.
FINANCING:
The Department will manage grant expenditures within existing appropriations and will evaluate the need to seek an increase at mid-year for Fiscal Year 2026-27, and if necessary, will bring the appropriate actions before the Board for approval.
