RECOMMENDATION:
It is recommended that the County of Monterey Board of Supervisors:
a. Approve and authorize the Director of Health Services or their designee to execute Amendment No. 1 to Agreement with Nurse-Family Partnership to add services related to helping low-income, first-time mothers achieve healthier pregnancies, improve parenting skills, and enhance the well-being of their children, leading to greater economic self-sufficiency, effective February 1, 2025 through May 31, 2026; adding $14,000 for a total contract not to exceed amount of $214,000; and update the Business Associate Agreement; and

SUMMARY DISCUSSION:
County of Monterey Health Department Public Health Bureau's Nurse Family Partnership (NFP) is an evidence-based program that pairs a nurse with a first-time parent to provide support starting in pregnancy and continuing until the child is two years old, supporting parents to achieve their own goals so safe, strong, and thriving families prosper. Since January 2019, 187 families have enrolled in the County of Monterey’s NFP services, and 39 families have graduated when the child turned 2 years old.
This Amendment will update the Business Associate Agreement due to the addition of insurance coverage tiers to the County’s BAA template. The County and Contractor seek to update the Business Associate Agreement, which conforms to the County’s current Business Associate Agreement template. This Amendment also seeks to expand the current NFP program to include participation in an NFP Initiative known as NFPx. NFPx allows for enrollment of individuals after 28 weeks gestation and individuals with previous live births. NFPx will fill the gap of first-time mothers who might not seek care until after their 28th week of pregnancy or are not interested in a home visiting program until the 3rd trimester. Expanding eligibility will also allow clients who previously did not meet criteria and have multiple complex issues to receive an evidence-based intervention.
This work supports the County of Monterey Health Department 2025-2028 Strategic Plan Goals: 1. Build community power and partners’ capacity to increase equity and improve health. 2. Provide exceptional person-centered care through accessible, community-focused health services. It also supports three of the ten essential public health services, specifically: 2) Investigate, diagnose, and address health problems and hazards affecting the population; 3) Communicate effectively to inform and educate people about health, factors that influence it, and how to improve it; and 4) Strengthen, support, and mobilize communities and partnerships to improve health.
OTHER AGENCY INVOLVEMENT:
The Office of the County Counsel and Auditor-Controller have reviewed and approved Amendment No.1 as to legal form and fiscal provisions, respectively.
FINANCING:
This Amendment is funded by the grant California Home Visiting Program. Health’s FY 2024-25 Adopted Budget (001-4000-8124-HEA003) includes sufficient appropriations for this Agreement.
