a. Approve and authorize the Director of Health Services or designee to execute an Agreement with the County of Santa Cruz, for the provision of administrative and fiscal intermediary services by Santa Cruz County as Host Entity for the County-Based Medi-Cal Administrative Activities (“CMAA”) and/or Targeted Case Management (“TCM”) programs, for the period retroactive from July 1, 2025 through June 30, 2028, for the payment to Host Entity of a one-time fee of $500 for membership in the CMAA and TCM Local Government Agency (“LGA”) Consortium, and annual compensation to the Host Entity in an amount to be determined based on the approved Consortium annual budget; and
b. Approve and authorize the Director of Health Services or designee to execute up to three (3) future Amendments to this Agreement that do not significantly change the scope of work.

RECOMMENDATION:
It is recommended that the County of Monterey Board of Supervisors:
a. Approve and authorize the Director of Health Services or designee to execute an Agreement with the County of Santa Cruz, for the provision of administrative and fiscal intermediary services by Santa Cruz County as Host Entity for the County-Based Medi-Cal Administrative Activities (“CMAA”) and/or Targeted Case Management (“TCM”) programs, for the period retroactive from July 1, 2025 through June 30, 2028, for the payment to Host Entity of a one-time fee of $500 for membership in the CMAA and TCM Local Government Agency (“LGA”) Consortium, and annual compensation to the Host Entity in an amount to be determined based on the approved Consortium annual budget; and
b. Approve and authorize the Director of Health Services or designee to execute up to three (3) future Amendments to this Agreement that do not significantly change the scope of work.
SUMMARY/DISCUSSION:
The CMAA/TCM Program is managed by the Local Governmental Agency (LGA). The County of Monterey Health Department (MCHD) serves as the LGA for the County of Monterey. The LGA draws down Federal Financial Participation revenues for CMAA and TCM claiming units on behalf of the County of Monterey. Services for this program include case management, referrals, and program planning for Medi-Cal services and outreach.
The CMAA/TCM LGA Consortium (Consortium) is a collaboration of Local Government Agency CMAA/TCM coordinators who meet regularly and pursue the proper and efficient administration of the CMAA and TCM programs. The Host Entity is responsible for collecting and disbursing funds for CMAA and TCM trust funds, paying the Department of Health Care Services (DHCS) for the CMAA/TCM administrative costs, paying the CMAA/TCM LGA consultant costs as well as managing and overseeing all procurement activities on behalf of the LGA Consortium. Santa Cruz County has taken on the role of Host Entity and will manage these duties. Approval of this agreement will allow participating programs to continue receiving funds. The County of Monterey has been receiving the benefits of this Agreement, this Agreement is to memorialize the informal Agreement and conduct of the parties.
This work supports the County of Monterey Health Department 2025-2028 Strategic Plan Goal(s): 4. Enhance Employee Wellbeing and Workforce Capacity. It also supports the following of the ten essential public health services, specifically: 1. Monitor health status to identify and solve community health problems. 4. Mobilize community partnerships and action to identify and solve health problems. 5. Develop policies and plans that support individual and community health efforts. 7. Link people to needed personal health services and assure the provision of health care when otherwise unavailable.
OTHER AGENCY INVOLVEMENT:
The Office of County Counsel and the Auditor-Controller have reviewed and approved the Agreement as to form and fiscal provisions, respectively.
FINANCING:
This Agreement is funded by Health Realignment and MAA revenue. Appropriations are included in the Health Department's (HEA014-8438) FY 2025-26 Adopted Budget. For the FYs out, provision will be made in the Requested Budget for the appropriate year.
