a. Approve and authorize the Contracts/Purchasing Officer to sign Amendment No. 1 to Agreement No. A-12642 with San Benito County in the amount of $186,164 per year, for a revised total amount not to exceed $1,116,984, for the revised full term of October 1, 2013 to September 30, 2019, to continue services in San Benito County that assist Supplemental Nutrition Assistance Program-Education (SNAP-Ed) eligible consumers in adopting healthy eating and physical activity behaviors as part of a healthy lifestyle; and
b. Authorize the Contracts/Purchasing Officer to approve three future amendments to this Agreement where the total amendments do not exceed ten percent (10%) $18,616 of the original contract amount and do not significantly change the scope of services.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Approve and authorize the Contracts/Purchasing Officer to sign Amendment No. 1 to Agreement No. A-12642 with San Benito County in the amount of $186,164 per year, for a revised total amount not to exceed $1,116,984, for the revised full term of October 1, 2013 to September 30, 2019, to continue services in San Benito County that assist Supplemental Nutrition Assistance Program-Education (SNAP-Ed) eligible consumers in adopting healthy eating and physical activity behaviors as part of a healthy lifestyle; and

SUMMARY/DISCUSSION:
California Department of Public Health provides funding to the County of Monterey as the lead agency to provide oversight for the subcontract with San Benito County. San Benito County staff that will provide nutrition and physical activity education to eligible community members, provide approved media messages and collaborate with community members and organizations to adopt policy, systems and environmental changes. San Benito County will enter into agreements with their local schools to provide services in accordance with the agreement between Counties. San Benito County staff will also attend Nutrition and Fitness Collaborative meetings and continue to co-host their Wellness Collaborative. San Benito County will complete the activities as outlined in the work plan annually. This Amendment No. 1 will provide continued services in San Benito County that assist eligible consumers in adopting healthy eating and physical activity behaviors as part of a healthy lifestyle. Amendment No. 1 will increase funding, term, and scope of services of this Agreement. All other terms and conditions remained the same.
The start date of the Amendment is retroactive to October 1, 2016. The Amendment was sent to San Benito County for approval by the San Benito County Board of Supervisors, which occurred September 27, 2016. Signatures from the San Benito County Officers were obtained following the San Benito County Board of Supervisors approval.
This action will support the Monterey County Health Department’s 2011-2015 Strategic Plan Initiatives: 1. Empower the community to improve health through programs, policies, and activities; 2. Enhance community health and safety by emphasizing prevention; 3. Ensure access to culturally and linguistically appropriate, customer-friendly, quality health services.
OTHER AGENCY INVOLVEMENT:
County Counsel and the Auditor-Controller have reviewed and approved this Amendment No. 1 as to legal form and fiscal provisions respectively.
FINANCING:
This Agreement is fully funded by the California Department of Public Health. There are sufficient appropriations included in the Health Department’s Public Health Bureau (HEA003-8124) Adopted Budget for Fiscal Year 2016-17.
