Title: a. RECOMMENDATION:

It is recommended that the Board of Supervisors:

a.  Approve and authorize the Director of Health or the Assistant Director of Health to execute Amendment No. 1 with Sun Street Centers for the provision of Whole Person Care Access Services for transportation and accompaniment of Whole Person Care enrollees to medical, mental health, social services, and housing services appointments, extending the term of the Agreement by seven (7) months for a revised full term from January 1, 2021 to December 31, 2021 and adding $123,166 for a total Agreement amount of $211,056; and

SUMMARY/DISCUSSION:

The County of Monterey Health Department (“Health”) administers the WPC program that coordinates physical health, behavioral health, and social services for vulnerable Medi-Cal beneficiaries who are high users of multiple systems and continue to have or are at risk of poor health outcomes. WPC services must additionally ensure that WPC enrollees receive crucial social-emotional support and advocacy while they attempt reintegrate with health care providers, housing, and employment.

 

At the direction of the WPC Supervising Public Health Nurse, the services to be provided through this agreement are (1) working collaboratively and at the direction of WPC staff, (2) providing transportation and accompaniment for WPC enrollees to health and social services appointments or other agencies and organizations as requested by WPC staff, and (3) assisting enrollees with the completion of applications and registrations to facilitate their access to housing, social service benefits, training, and employment. 

 

The WPCAS, having familiarity with characteristics of homeless people, will work with chronically homeless persons who are well-adapted to living outside of mainstream social norms. In their role of Access Specialist, the WPCAS will act as an advocate for the WPC enrollee to agencies, service providers, landlords, and other entities while guiding them in overcoming barriers to wellness, homelessness, and independent living. The WPCAS may be asked to assist WPC enrollees in completing rental application, housing discrimination, or eviction forms to facilitate their access to housing and homeless services. The WPCAS will not directly provide healthcare services or mandate the activities of the enrollees assigned to their aid.

 

This work supports the Monterey County Health Department 2018-2022 Strategic Plan Initiatives: 3. Ensure access to culturally and linguistically appropriate, customer-friendly, quality health services. It also supports five of the ten essential public health services, specifically: 3. Inform, educate, and empower people about health issues, 4. Mobilize community partnerships and action to identify and solve health problems, 5. Develop policies and plans that support individual and community health efforts, 6. Enforce laws and regulations that protect health and ensure safety, and 7. Link people to needed personal health services and assure the provision of health care when otherwise unavailable.

 

OTHER AGENCY INVOLVEMENT:

The office of County Counsel and the Auditor-Controller have reviewed and approved this Agreement as to form and fiscal provisions, respectively. 

 

FINANCING:

This Agreement is funded with Whole Person Care funds.  There are sufficient appropriations in the Health Department (HEA014-8438) FY 2020-21 Adopted Budget and will be included in the Health Department FY 2021-22 Requested Budget.
