Title: a. Title

a. RECOMMENDATION:

It is recommended that the Board of Supervisors:

a.                     Approve and authorize the Director of Health Services or designee to execute a two-year Facility Use Agreement for the term November 1, 2023, to October 31, 2025 with Aspire Health Plan, A Nonprofit Corporation (Aspire) to use office space on County-leased premises located in Salinas, Seaside and Marina California to provide Pediatric Wellness Program sessions to eligible Clinic Services’ patients at no cost; and

b.                     Approve and authorize the Director of Health Services or the designee to sign up to three (3) future amendments to this Agreement where the amendments do not significantly change the scope of work, and with no exchange of funds.

 

SUMMARY/DISCUSSION:

The Health Department Clinic Services Bureau (Clinic Services) operates ten community clinic sites designated as Federally Qualified Health Centers (FQHCs) which provide preventive, primary, and specialty medical care services.

 

Aspire Health Plan, a nonprofit corporation, is part of the Montage Health family of services and offers programs and services to promote wellness, improve care and help find solutions to overcome barriers to good health. Aspire’s Pediatric Wellness Program offers free pediatric wellness coaching services through group education and one-on-one sessions for families referred by their child’s pediatrician. The program includes group learning on goal-setting, healthy eating and physical activity. Sessions are facilitated by a pediatric wellness coach with the primary goal of helping families create lifelong healthy habits. Clinic Services has suitable clinic space and equipment to support the Pediatric Wellness Program and is willing to allow Aspire to utilize such facilities to educate its patient population about nutrition and creating healthy lifestyle habits.

 

Approval of the recommended action will allow Aspire to deliver on-site Pediatric Wellness sessions directly to Clinic Services’ patients in locations that are familiar and accessible to them, while supporting County of Monterey's efforts to promote a healthy lifestyle to better prevent and reduce the risk of obesity and diabetes in our community.

 

This work supports the Monterey County Health Department 2018-2022 Strategic Plan Initiative(s): 3) Ensure access to culturally and linguistically appropriate, customer-friendly, quality health services.  It also supports one or more of the ten essential public health services, specifically: 3) Inform, educate, and empower people about health issues, 4) Mobilize community partnerships and action to identify and solve health problems, and 7) Link people to needed personal health services and assure the provision of health care when otherwise unavailable.

 

OTHER AGENCY INVOLVEMENT:

County Counsel and the Auditor-Controller have reviewed and approved the Agreement.

 

FINANCING:

There are no fiscal provisions included as part of this Agreement.
