b. Authorizing the Director of Public Works to execute and record the Notice of Completion for the Lonoak Road Bridge at Lewis Creek, County Bridge No. 402, State Bridge No. 44C-0158, Project No. 11-220565, State Project No. EA 05-927094L, Federal Aid Project No. BRLOZB-5944 (035), performed by MCM Construction, Inc.

RECOMMENDATIONS:
It is recommended that the Board of Supervisors adopt a Resolution:
a. Accepting the Certificate of Completion for construction of the Lonoak Road Bridge at Lewis Creek, County Bridge No. 402, State Bridge No. 44C-0158, Project No. 11-220565, State Project No. EA 05-927094L, Federal Aid Project No. BRLOZB-5944(035), performed by MCM Construction, Inc.; and
b. Authorizing the Director of Public Works to execute and record the Notice of Completion for the Lonoak Road Bridge at Lewis Creek, County Bridge No. 402, State Bridge No. 44C-0158, Project No. 11-220565, State Project No. EA 05-927094L, Federal Aid Project No. BRLOZB-5944 (035), performed by MCM Construction, Inc
SUMMARY/DISCUSSION:
On June 14, 2011, the Board of Supervisors awarded a contract to MCM Construction, Inc., for construction of the Lonoak Road Bridge at Lewis Creek, County Bridge No. 402, State Bridge No. 44C-0158, Project No. 11-220565, State Project No. EA 05-927094L, Federal Aid Project No. BRLOZB-5944 (035) (Project). The Project consisted of construction of a new two-lane bridge on the existing alignment, a temporary detour adjacent to the existing bridge, approach embankment grading and paving, removal of the existing bridge, and associated improvements on Lewis Creek and Lonoak Road. The contract is now complete in accordance with the Plans and Special Provisions. The work is ready to be accepted by the Board of Supervisors.
OTHER AGENCY INVOLVEMENT:
None.
FINANCING:
The total Project cost, including engineering, environmental, and construction, is $3,020,814. The Project was financed by Federal Highway Administration Highway Bridge Replacement and Rehabilitation Program Funds, State Seismic Retrofit Funds and County Road Funds.
