SUMMARY DISCUSSION:

The County of Monterey CCS program provides diagnostic and treatment services, medical case management, and physical and occupational therapy services to children under age 21 years with CCS-eligible medical conditions. Examples of CCS-eligible conditions include, but are not limited to, chronic medical conditions such as cystic fibrosis, hemophilia, cerebral palsy, heart disease, cancer, traumatic injuries, and infectious diseases producing major sequelae. CCS also provides medical therapy services that are delivered at public schools.

As a Whole Child Model Designated County, the CCS program in Monterey County is administered as a partnership between the Health Department, the California Department of Health Care Services (DHCS), and CCAH. Services are funded through a combination of federal, state, and local funding. County of Monterey CCS program staff determine an individual’s eligibility for CCS services. Once an individual is deemed eligible, CCAH provides case management to individuals that are CCAH members, while the County of Monterey CCS program provides case management to CCS eligible individuals that are not CCAH members.
