RECOMMENDATION:
 
It is recommended the Board of Supervisors:
 
a.      Award a contract in the total amount of $35,875.00 to Tombleson Incorporated, the lowest responsible and responsive base bidder, for the Natividad Medical Center (NMC) ED Exterior Door Replacement Project No 9600-28 Bid NMC-1003; and
b.      Approve of the Performance and Payment Bonds executed and provided by Tombleson Incorporated; and
c.      Approve a contingency (not to exceed 10% of the contract amount) to provide funding for approved contract change orders; and
d.      Approve the Purchasing Manager at NMC to execute the contract and subject to the terms of the Public Contract Code, approve change orders to the contract that do not exceed 10% of the original contract amount, and do not significantly change the scope of work.
 
SUMMARY/DISCUSSION:
 
Building 500 is occupied by the Emergency Department, Radiology Department and Admitting. There are two automatic entrance doors allowing access to these departments and provide patient access to the Emergency Department and Admitting. These doors also serve as an entrance for NMC staff. The entrance is comprised of an outer door which is a bi-folding automatic door and an interior door which is a pair of automatic swing doors. The Bi-fold doors have been out of operation for two years. Their failure was caused by exposure of high wind pressure, preventing the doors from opening and causing an actuator failure. The double swing automatic doors, although still functional, have also been a casualty of the exposure of high wind pressure. These doors do not meet current safety standards (ANSI 156.10 - Safety Sensors for closing on approach and entrance) and are at the end of life. Typical life span of swing type automatic doors is 10 years, and these doors have been in operation since the opening of Natividad Medical Center in 1999.   The Emergency Department has requested to have the automatic doors replaced with automatic sliding doors.
 
During Fiscal Year 2011-2012 Natividad Medical Center's Capital Committee approved the funding to replace two automatic entrance doors in building 500. Natividad Medical Center selected the architectural firm Wald Ruhnke and Dost from Monterey County's Master Agreement (MA*21) to provide drawings and specifications for the Emergency Department Exterior Door Replacement project. The architect submitted construction documents to the Office of Statewide Health Planning and Development. The plans were approved and permit has been issued.
 
NMC advertised ED Exterior Door Replacement project on August 6, 2012 and on August 30, 2012 at 2:00PM received two bids for the Emergency Entrance Doors project. After review of the Bid documents, NMC proclaimed Tombleson Incorporated, as the lowest responsible and responsive base bidder.
 
OTHER AGENCY INVOLVEMENT:
 
County Counsel has reviewed and approved this Agreement as to legal form and risk provisions.  The Auditor-Controller has reviewed and approved this Agreement as to fiscal provisions.
The project has been approved by the NMC Finance Committee, NMC Board of Trustees. The project has been supported by the Capital Improvement Committee on October 1, 2012. . The project has been supported by the Budget Committee on October 31, 2012.
 
FINANCING:
 
The total agreement cost of $35,875.00 for construction, and $3,588.00 for the 10% contingency, is $39,463.00. Natividad Medical Center's capital budget for Replace ER Doors is $32,500 and is approved in the Fiscal Year 2012/2013 Capital Budget #B-2011-49. The remaining capital deficiency of $6,963.00 will be augmented from Building Wide Upgrades Capital Budget # B-2013-1
